My mother has fell twice in night activities room. She has fell six times from her locked room. Room has not been secured so a man stood overtop her in his underwear. No aid is trained regarding oxygen usage so they ignore when oxygen units may be non working. They do not check them. No aid is professionally trained to give patient care. No STNA credentials. My mother has had food dropped off and no assistance with eating and she needed it and some nurses were aware. I have had to fold her clothes and put them away when this was supposed to be done by facility. I found toilet not cleaned with residue left on commode seat. I have found a washer smoking profusely and no technician or management was called let alone fire dept. I have found portable oxygen NOT operating and I found the concentrator in her room not ON and also not working on another occasion. Since I have complained, I have been told that I am not allowed on premises. I will be reporting to ODH, office of Aging and the AG.

Poor management

Our meals at the Clarendale at Indian Lake in Hendersonville Tennessee come with free drinks, and free desserts. At night our meal includes a free dinner roll which very seldom is served. Getting the drinks is no problem, but getting served to drinks is a big problem. We reset 20 minutes last night waiting for a server to ask us if we wanted desserts They were more interested in cleaning off tables. So we left. This is an every day occurrence. It’s a shame such a nice place could be so poorly run.

Don’t bring your loved one here

disappointed in harbor village. I would not recommend this place to anyone. You’re better off leaving your loved one at home to fend for themselves and save you the money and increase rates. There team and employees look all burned out tired and walk around stressed and unhappy to be there. For the money you invest in a company you would think you would hire more help during the enevings. But it’s nice to know they are redoing all the carpeting because the building looked old nice to know that’s where the money is going instead of to the extra care staff and nurses on the weekend and evenings they need due to all the questionable things happeneding here. Save the money don’t do it.

DO NOT SEND ANYONE POST HOSPITILIZATION

NEGATIVE STARS. You will be told the Clare is one of the nicer facilities and it may be true for the nursing home portion however I have nothing nice to say regarding their rehab center. I met [name removed] the case manager to tour the facility and discuss what my mother had been through and the level of care she required. I was told the Clare was more than capable of handling her needs however that was a blatant lie. This is NOT an adequate rehabilitation facility for post hospitalization patients. The staff is under trained, under qualified and under staffed. I was incredibly clear on my mothers condition and the level of patience and care needed to keep her comfortable. Not only did they not respect our wishes; they did the exact opposite. They bullied my mother and when she asked them to please be more gentle with her and to stop hurting her she was told if she didn't like their treatment she could go back to the hospital. The short term rehab is incompetent.

My father entered short term rehab from a fall for 17 days in [date removed]. In 17 days this facility managed to lose not one but both of his hearing aids. The value of these hearing aids was 12k. When you register the facility requires you to list the patient's property and value of the items. Cypress Village made no effort to safely store my father's hearing aids. Both of the hearing aids were not lost on the same day they were different days. This facility made no effort to learn and prevent the loss of the second one after the first one was lost. Part of my father's cochlear implant was found in the dryer. The other parts and the second hearing aid were never found. I placed signs in his room with pictures of what the hearing aids looked like for the staff to make sure they had not fallen off in the bed prior to changing the bedding. My father was unable to get out of the bed to charge and store his own hearings aids. This facility took no responsibility for this loss.

Questionable Stay for my MOM

My mother suffered a pelvic fracture and was sent by the hospital to this SNF. Mom was in a lot of pain and at first I thought it was her because of her injury. There were good doctors who were looking into the orders for her care. They don't see the poor care that several CNAs on the floor failed to provide. I hired a private CNA to be in the room with my MOM. The CNA had to be behind the nurses to get them to do diapir changes, otherwise my MOM would have sat in her poop and urine for hours.They also didn't post her orders on the board she was there a week and half and was showered twice. They roller her like a rag and she stayed in pain. I had to bring it to the attention of the SNF Administrator and he sought action for my MOM. We ended up pulling her from the SNF and the social worker helped us get a referral to the [removed] to provide Home Care Pt. It was not a good experience and because of their inconsistent practices, my MOM will never go to another home.

Continually short-staffed in the medicine administration area, causing many late med administrations. My mother, who has chronic pain just went through a half-day of distress because her morning medications were not administered to her. I was on site at the facility at the time and found that there was no nurse on duty, and only one med tech who was overwhelmed and referred me to Management. This ongoing issue is the responsibility of Management to address and correct, and I have gotten no response from top leadership when inquiring about it. This should not be happening given what my mother is paying to live here and obtain the staff services.

EPIC FAILURE

The Delaney lulled me into making the worst decision for my 94 year old mother. In the two months she lived there, she was treated with disrespect. She did not receive the level of care The Delaney told us they would provide and for what we paid for. I had to bring in private help to assist her. The staff neglected her because she was needy. It boarded on elder abuse. What was really telling was when we told The Delaney we were removing my mother, we did not receive one phone call from The Delaney as to why? I am considering filing a complaint with the State.

Caution

ML’s selling point is that there’ll never be a reason for you to have to move out. Not true! Once my mother was diagnosed with Alzheimer’s, and there was no care in place for her due to a 22+ resident waiting list at The Springs, she was forced to move into a new community. ML charged a penalty fee of 3 months rent. The value of her apartment had increased considerably since she moved in during the recession so they were making a lot more money by her leaving and still they refused to waive the penalty fee. I asked the ED to explain how they could justify doing so, and to specifically state what alternatives of care they would have made available to her while she waited a year or more to get into the on-site memory care. I’ve been waiting for a response since [date removed]. ML also tried 3x to charge us [fee removed] to replace my mother’s carpet and paint her apt when she was on the plan to have it all paid for by ML that same year. Profits over resident care.

Avoid memory care! And, don't trust their advice!

My mom suffered there for 3 weeks and we got her out as quick as possible. It was a costly mistake in that they did not refund us any of the set up fee. For the 1st 8 days she was put in isolation(Covid) and given cold food 3 meals a day, food was better when my dad could take her to the main dining room.

24 hour care is a lie!

Sale department sells homes as 24 hour care and it’s not true after nurses leave cna or caregiver decide what is good for the resident. After dinner you can not find a single soul in building 1 and 2. I walked over to building 1 for help and there is no one the front desk doesn’t know where people are at. The girl I found sitting on her phone on the couch with legs up. They always have new people working or people from temps working because no one stays long enough to know there name. The leadership is nice but seems like they find excuses why they can’t keep staff working in there entire campus.

This place is just Horrible

The staff is unhappy. Everyone is over worked. The care team is stressed out and miserable. The food is horrible. Not enough staffing in any dept. Activities are non existent. The place is filthy. Lots of new faces which says the staff turnover rate is unacceptable. The weekends are abominable here. Visit this place on the weekend to get a real sense of what goes on. Minimal staffing on the weekends. No weekend supervision. Highly recommend going to the new [removed].

Poor and medical services is a concern as managed by LCS which revenue is 56B

The interior design is nice, food seems tasty ideal location in Towson! Blakehurst it is more of a hotel than a community. The entrance way has oCaters to a bridge&Country Club group which is unfortunate as not everyone belongs to country clubs and/or plays bridge. Trying to get through to my Mom via the Blakehurst front desk it took over 1 hour. I was transferred to 5 different departments, on one occasion hung up on, then come to find out some of the medical services staff assumed the new director of Blakehurst was a female with the name, [name removed]. I cannot image if it was an emergency and had to reach Mom right away. With a LCS,Continuum of Care company making $[removed] in revuene, the Blakehurst monthly costs of approximately $[removed], my Mom has to eat $[removed] a day in food, a 30 minute exercise program plus a movie. Recently, the repaved the entire asphalt parking and roads which was not necessary. Added a flat screen TV to the mail room and are undergoing lots of renovations, new pool

Do not recommend due to complete lack of communication and proper care of resident.

If someone is looking for good communication, this is the last place they should go. There is no communication at all unless you nag at them continually. They really need to improve on the overall care honestly. They present well at first, but then as soon as money changes hands, that seemed to be all they cared about, not the loved one placed there.

would not recommend for Memory Care

my mother has been a resident of Havenwood of Minnetonka for about 7 months. She had lived in another memory care facility for a little over a year before moving her to Havenwood. The sales staff was very attentive to answering all my questions and the facility is lovely however the care is not. I would not recommend Havenwood for the following reasons. - Lack of communication regarding my mothers condition and/or care - difficulty in communicating with memory care staff - Lack staff engagement with residents - Staff is not very attentive to needs of residents

Compassion is needed

The caretakers in memory care at Clarendale are not adequately trained to truly be able to care for the different stages of dementia. They want one size fits all and that just is not the case with dementia patients. Most (except for 2 or 3) of the caretakers don't have the compassion it takes to truly make the person feel safe regardless of what stage they are in. Unfortunately we learned this after getting our loved one into another facility where there was no question about how much they cared. Knowing how to handle different situations makes all the difference and I only wish we had made the switch sooner. My recommendation is to provide as much training up front as well as o going training for every person who I tracts with those with dementia.

The sales people will promise you things that the organization has no intention of providing. When I commented on things like - I was told my mom would be able to choose between two meals - they said - sorry you were told, that is not true. I also come and observed activities before deciding - again - was told (and saw) residents could pick between 2-3 activities at any time. Also not true - there is one activity at time and it usually sitting and watching tv or listening to music on tv. They do have stretches but there is no researched methods to help memory - which I was also told. Also - do not believe that they have any true memory care programming like heartfelt connections (mentioned by sales person). They didn't even know what it was when I asked. Again - do not trust whatever the sales people tell you. Also - I suggest putting a camera and remote thermostat in the room so you can monitor that. My mom AC broke and the room was 88 degrees when I came to visit. No one noticed.... then they out a temporary unit in and it heated up her room. I called to ask them to check on her and they said it was fine. I didn't believe them - came and it was 89 degrees in the room. I now monitor the temperature remotely (AC broke again last week / saw immediately on thermostat and complained). If your loved one can take care of themselves - this place is fine - as you don't need care. If they cannot - either put in cameras and thermostats so you can watch them all the time - because these folks don't. (Note - when asked how often they would check on my mom - sales people said all the time. Director said we can't set times because then we are a nursing home.... so they don't check). They do come at 10pm - waking mom up, turn on all the lights and pick up garbage. Also - just for fun - when my mom moved in she did not receive a shower or have laundry done for over a week until I complained. Beware of this place.
