Five Star Senior Living

This place has a sign on the front door that says they are a dementia friendly pace. My dad was not even three two months before he eloped. And somehow that was my fault, and I needed to get him a new place immediately. However, they knew ahead of time that he had his days and nights mixed up, and he roamed around at night. I still have not received a refund of rent I had paid ahead of time. Also, he was only there six weeks, and they did not refund his community fee back either, after THEY requested that he leave. I liked the nurse, some of the caregivers, some of the servers, and the secretary. But I cannot give them a good rating. The bookkeeping is way behind. They're understaffed, I'm assuming. Dealing with them it's like throwing Jell-O at the wall. I would not recommend it.

This had an older feel during our tour.

Our loved one was often left in the bed for days, sometimes sitting in urine. It was such a bad and disappointing experience. It started out great, we loved the sales person who was working with us in the beginning, however the care services need major improvements. The doctor on sight could never figure out the correct medication for our loved one, the bathrooms there were fifthly and our loved one was rarely changed into clean clothes. They were not even equipped to handle our loved ones basic cleaning needs. They were never able to follow through with any promises they made to our family

We toured Fox Hollow and it was during covid times, but it was very depressing.

Double-check the details before you move in. I do not feel they gave us all the info we needed to make a real decision. I would not recommend this community to others based on my experience.

The community is very expensive and raised our rate right away. They are also not very attentive. I would not recommend this community others. Communication was not good and

I needed a location that would allow my service dog and 2 emotional support dogs. When I signed my 2 month lease I stated several time I had my three dogs and was reassured that wouldn’t be a problem. But in the dining area they didn’t want my service dog there. I had to tell them the law. Socialization, activities none. Maintenance never helped with my carpet. I had a fall and spent several days in the hospital. Upon return I spend tine in the medical unit. There I witnessed staff belittling 2 residents and treating me like a leaper. They required I take a COVID test, but 2 months I am still waiting. And I’m waiting for my COVID injection. And was told I had to get rid of my dogs. I came because I was told I could have my three dogs. Staff that delivers my meals are ok if you like your drinks spilled on the tray. Staff that deliver the mail are friendly. Can’t get involved in any activities. I came so I could keep my 3 dogs I love the location but,

They rased our fees each month and the billing was always incorrect. The head nurse was fantastic, but the community was not well run. I would not recommend this community to others

Due to the bug I would say not to come to this community. They've cut down on activities that there is hardly anything for us to do. The food isn't good. I'm sorry I'm here. They are allowing us to eat in the dining room again. The room is too small for me, and has a tiny kitchenette.

I would not recommend this ‘5 star’ community. Highly disappointed for the incredible amount of money my father spends every month. My father has been at Forwood for 3 months, he moved here after my mother died. He was very lonely and we thought this would be a great way for him to meet people. Covid is still going on and this facility does nothing to help the seniors stay active and provides NOTHING for to do to interact safely on a daily basis. After a storm over a month ago, the front entrance is still in disarray and no work has been completed to fix the entire front entrance and adjacent meeting area for guests ‘who are not allowed to visit’. My father wants to move and we are looking. You pay a 4000 dollar one time fee....you get nothing for this! Do not place your loved ones here...it is all a lie to get your money!

At first we were pleased with Summit Place of Southpark, but as time went on we were not pleased. We had some issues such as medication not given properly on several occasions or medication mix ups. It did not seem like they were monitoring it close enough. When we went to pick up my mothers things, they had sat them outside the facility. Her stuff was waded up and thrown in open boxes and there were also belongings of hers that were missing. 6 weeks after she passed, we found out the facility had not given my mother the cards that people were sending to her. I would not recommend Summit Place.

Many issues at Five Star. They are understaffed. Staff are not adequately trained. Management & corporate are totally unresponsive to resident needs. Food is poor--not nutritious, presentation is awful, usually over-cooked & not enough variety. Wait time for meals is long. The relatively new computerized dining system has not made an improvement. In fact, it's worse because servers aren't properly trained. The dining room manager isn't not on the floor supervising staff. If Five Star dining services were a restaurant, they'd be out of business in a flash. Activities are the same things repeated. There's no creativity in programming. Widespread boredom & depression among residents are ignored. This is critical. Residents justifiably feel no one cares or listens. There is no value for the high price. Go elsewhere.

After a month in this facility, my family and I have become very disappointed in the nursing care provided and the administrative staff for not listing and providing information as to decisions they make. Upon admitting him the staff welcomes you with open arms, and assures that this is a great place to be. We our family visit him every day, several times a day. and as weeks passed we noticed the lack of care being offered to him and other senior citizens. Lunch staff complaining about there pay in front of visiting family members. Patients in distress, (respiratory) and waiting 15 minutes up to an hour for nursing staff to respond after the nurse call button was pushed. On many occasions they stop speaking English so you can not under stand them. My father unfortunately can not afford a private room and can not afford to pay for extra amenities. I urge every one to pay close attention to what is going on around your loved one, and I hope the standard of care provided is not based on a monthly check. I have no choice but to look elsewhere for better care. Please note: that there are and have been some caring staff, but unfortunately were not involved with every day nursing care.

My relative lives at the Forum in assisted living. The Forum is not a clean facility and I do not feel that the care is adequate. The apartment unit always smells badly. We try to maintain cleanliness in the apartment and we ask the staff to assist as well but they do not. The unit continues to be dirty and smelly. The staff launders clothes that often we never get back. Instead of actually cleaning the bathroom, they will wipe here and there with a clorox wipe for one minute and leave the room. I do not recommend this facility for your family member if he or she requires a lot of help. We are investigating other living arrangements. The independent living area is probably fine as a resident can do more for themselves and are more aware. As a positive, I do think that the activities presented to residents are fun and engaging. As a whole, from what I see, the staff the friendly.

I was promised things that they could not give my family member. The building is not clean. Hot meals are not always available. We were only there a short time, and would not go back. Some of the staff are rude. Sorry. I was disappointed.

I have a loved one here. My main frustration is the heating & cooling. The air conditioning was out this past weekend (fortunately it's spring, May/2019). The heat goes out in the winter on occasion, too. Happens EVERY season for over 4 years now. I understand they run on an old boiler type system and when something happens, I guess older parts are hard to get. Residents are given fans & space heaters, but it's not acceptable. No A/C last year with temps in the 90's for residents is unacceptable and not just a day or two. They need to permanently fix this. It's NEVER a quick fix and is just unacceptable. Otherwise a good facility.

Friendly staff & residents. Didn’t feel like a fit for my mom needs right now. Location wasn’t ideal for us. Older building & found rooms needed updating (which they were in process of doing).

Five Star presented as an Independent Living facility that has minimal cross over with their Asssted Living and Memory Care units. It seems to us that because IL is so much cheaper than AL and MC that new residents with full time aids and mobility devices as well as memory issues are being admitted to the IL section when they don’t seem to match well with the truly IL residents. Next, we have had many negative interactions with the head of maintenance who has actually been quite verbally rude and unlike many of the other staff, doesn’t acknowledge me or my loved one when passing in the hall, and doesn’t make sure a full elevator of residents can enter without the door closing. After a conversation with the director, it was made clear that their working relationship is more important than resident satisfaction. Finally, we did not find or choose Five Star with any help from a Place for Mom. Their system is free because they won’t meet with you, they rarely call back, and won’t accompany you on site visits. I would not recommend APFM.

I’m being prompted by SeniorAdvisor to submit a review so I am doing so. I must first say that dad stayed at the Gardens during early 2018 and the Gardens had hired new staff to oversee needed changes at the time. I’d recommend you check the VA Dept of Social Services website for complaints filed to check status and whether care has improved this year. I checked the site too late, only after I had realized my first impression was egregiously wrong, and had to file complaints myself. There were open complaints being addressed and had been for some time. My main complaint was with the care and dispensing of medicine, which are crucial. Bathing, therapy, and medicine dispensing was nothing like they’d promised. Even with our daily presence and reminders, countless oversight and errors occurred. Communication among staff was poor with an inadequate system in place. The most heartbreaking were the errors and delays in med dispensing, and the running out of meds to the point where doses were skipped. They lost track of one med and it completely dropped off dad’s list. Dad ended up choking on meds bec they weren’t dispensed correctly and ended up in the hospital. It wasn’t long before he passed. This facility is probably great for a resident not needing much medical care. We paid extra for advanced level care but this place wasn’t prepared or equipped for it.

This ALF is worse. It is very very overpriced. Everything they do is extra. Staff is overworked. They work extremely long hours. They look exhausted and are grumpy. Resident care is very poor. Turnover of staff is a revoling door. Residents wait and wait for help and are incontinent as a result. Rooms are too small to accommodate 2 wheelchairs. Wicker furniture is not appropriate. It is never cleaned as under the bed is not either, Carpeting is well worn.
