My experience was with Memory Care. I would not recommend this facility to any of my friends. I felt the staff was not fully trained. They were often short staffed. I recognize that my mom required skilled care as well as memory care. This was not the facility for that. It was not a great experience for myself or my mom.

Well water, not city water. They spray for ants in the resident rooms. Because there are ants in resident kitchens. Emergency pull chains are only located in the resident bathrooms. Only some rooms have a walk in shower, most have a very high step in tub. Windows were difficult to open in the resident rooms. Only basic cable is included. Very limited channels. Nighttime front desk staff was not pleasant. Small dogs and cats are allowed-the hallways often smelled of animal pee/poop. Residents did not clean up animal feces in the grassy outside areas. Not enough outside seating. Washer/dryer room has restricted hours of use for residents. Very inconvenient. Need newer/more machines. Difficult to access building after 7 pm. No staff at desk to answer the doorbell. Sign in/out sheet is out of date--and inconvenient. Staff delivered/administered medications to residents. No defibrillator on premise. Staff has keys to every room No cameras in hallways= no visibility of residents except call button if a resident falls or enters another residents room. And no staff accountability when entering a residents room. Emergency back up lights failed once they ran out of charge leaving residents in the dark. Candles and flashlights were used during outages. No handrails along the hallway walls. Food is carb heavy. Dining service is slow. Not enough kitchen/wait staff. Hot food was often cold and unappetizing. Breakfast was bland and worse than a cheap hotel chain. Offerings were often out of stock. Same items every day. Took away hot food service that was offered 2x a week. Snacks are offered daily. Mostly sugary sweet items. Lobby TV was too loud. Staff encouraged residents to leave area by 8pm Always chilly. 72 degrees year round. Busing service was great in the beginning. Then became by advance reservation only Not many outings Very few activities

we had a horrible and unjust experience with the new executive director! the rooms are not well maintained. Dust bunnies everywhere. Slimy shower floors. there are some nice people there. there is a severe lack of empathy and understanding when dealing with people who have dementia I never tasted the food. there were several listed activities

Very expensive

This is a review on costs only because it is so difficult to actually get costs from any facility. Memory care for a private room will run $[removed] per month based on the information they provided to me. It ought to have all five star reviews for that amount of money. Not for your average resident.

Our Experience

My loved one was previously a resident at The Forum at Tucson. They are not really capable of taking care of a hospice patient. The staff were not knowledgeable enough to take care of my loved one's needs. They had to be told several times that she couldn't eat on her own. One of the male staff took her from her bed to give her a shower. My family member who was there told him we have [name removed] from Harmony that will give her a shower. He said he couldn't wait for her, against her request. She wanted to leave the room and tell someone, but he said she had to stay. The way he handled my loved one was totally humiliated and said she wanted to just die. I did not discuss this with anyone in charge because I was afraid they would take it out on her.

Staff (CNAs) were rude and dismissive of simple requests, very short with my mom. Housekeeping was fine, as far as I know. Communication was lacking, almost nonexistent. I was told I would get a call after move in, no call for 4 days and that was to tell me mom had escaped. The move in was not great, times to move in non communicated, no extra care seemed to be given during mom's transition, she was observed trying to exit 3 days, no one alerted me or the head nurse until after she escaped by climbing a wall twice her height. Lock the patio or have monitored cameras, a Memory Care facility should be equipped to deal with sun downing and wandering. It should not require me to hire a caregiver to sit with mom. I would not recommend this facility to others.

Only recommend for Individuals that are mostly Independent

Having many issues with my mother in law getting showers, room not being cleaned , broken closet door multiple times, lost her glucometer we brought in with her and then charged us for glucose strips and didn't notify us until a month after she was there that they didn't have her glucometer, not having our correct information in her chart even though we had given everything on arrival, not having the automatic bank withdrawal form in her chart that I filled out prior to her moving in causing much stress to my mother in law, not being able to contact staff and not receiving return phone calls when leaving messages. The cleanliness is a 2 for her room. Every time we go there we have to clean her bathroom. I had to request that her garbage be emptied more than once a week because the smell in her room was awful. Rest of the place seems clean. Staff are mostly friendly when we are there. However they are not responsive to my mother in laws needs. She seems satisfied with meals. she does not participate but there are a lot of activities. Paying mostly for her room and dispensing meds and meals. We don't really feel that she is getting great care

The memory care area needs a lot of work and better help! The area had an odor. It could have been cleaner. The food needs improvement. Over all the memory care area needs to be overhauled. We not recommend it to anyone. I will say the young girl that sits out front was very sweet and helpful but she can't control what goes on in the memory care area. I hope it improves.

We picked this community because they stated they we're building 4 PICKLEBALL courts, which my mom plays But, they have no programs in place/ no teaching pro, which they promised.[name removed] , the executive director of FountainView, is blaming the lack of any PICKLEBALL program on a missing fence.

our family experience

Based on the experience we have had so far with Five Star Premier we feel they could improve on all areas. To be honest we would not recommend this community to other families looking for a place for their loved ones. The place needs to improve especially in the overall quality of care for the residents

Disappointed

The first few months the care was exceptional. Unfortunately, changes are taking place within the facility, and the care has decreased. Employees do their best, but it begins at the top! Looking for other alternatives for our loved one.

My Tour Experience at Carriage House

Carriage House was a disappointing tour. The rooms were is disrepair and the closet doors were off the rails. We were surprised the community showed us the room like this. In addition the tour guide was talking down about another community in the area.

Our family had an in person tour with The Gardens of Port St. Lucie and It was not very clean, and smell was very bad. Turned us off in arrival. Room they showed us as example very nice. Room my step dad was to go to very old and run down and next to an exit door which he would have done.

I seemed to be a problem for the tour guide the day that I went to tour with The Forum at the Woodlands. She didn't think she needed to do any more tours and it wasn't her turn. The place just seemed older.

Pros: staff seemed engaged with the residents and they were nice. It was more cost effective price wise. Cons: The memory care floor was on the second floor - did not make us feel comfortable if there was an emergency. The memory care floor was also not very clean (floors were dirty and it didn't smell clean and didn't seem well kept). The rooms were my least favorite out of the 3 places - not as open and bright as the other two locations. There bathrooms seemed older and just not as well kept / clean. When we left Morningside, we all knew this was not where my mom would be placed.

Tour of Leisure Park

We toured Leisure Park during our search for Independent Living for our loved one. It was not as clean as the other facilities that we toured. They also didn't offer a very good variety of activities. It was kind of depressing, the residents were just hanging out in the hallways.

Not a place for Mom

I moved my parents here in December. Mom to the Homeplace and my dad in my house. Long story short if your family member is in assisted-living it’s going to be a pretty good life. If they’re in the memory care center expect that they want your parent submissive and quiet. They prefer their memory care residence to sit around a small table in wheelchairs. The quieter the better. People with dementia are not always that. They will use drugs to make that happen. After it was threatened that my mom would be locked up in a hospital for a psych evaluation for 72 hours, I brought her home. Yes she still has dementia. That’s not going to change. What has changed is that I’m not paying over $8,000 a month for very little care. Witnessed after I put a camera in her room that she was often left crying in her room during the day. No bed checks at night. Not two hours or anything else. Not a place for Mom. They don’t know how to care for someone with dementia.

A Sad Place for Mom

I regret moving my mother to Morningside and she is now living at home. I was most concerned that when my mother needed help she would get it. I was assured the call bell would be answered within 20 minutes. When that didn't happen, I met with the director who yelled at me, and said "Hmmmm" when asked to account for no one returning calls for help. The call bell works like this: ring it and then wait until a person who is managing a dozen or more others can get to you. Do not call for help during dinner service. That is their priority. If your loved one has fallen, they will have to wait. Do not expect toilet paper to be regularly stocked in the bathroom. Do not expect the staff people you see this week to be there the next. Although physical and occupational therapies were promised NONE were delivered in 3 weeks. They had staffing problems, I was told. The woman in the room next to my mother would sit in the hallway and cry all day, ignored by everyone.

Dropped the ball

They don't exactly have their stuff together. They were all about sales but once we signed the contract, they dropped the ball. They first claimed they never received the FL2 (push the blame to someone else.) But in reality, they wanted it phrased a different way, but no one had told me that. I had to scramble to get another one sent the day before my mom was to move in. I had been there multiple times prior to that day but no one had said a word about needing anything more for move-in. They pushed for me to sign at the end of February but I did not hear back from our attorney until March so the contract was actually signed on [date removed]. We did not receive the keys until [date removed]. I spent a week cleaning and organizing her space and she moved in on the [date removed]. Our first statement started with a prorated bill for February and all of March. I have emailed three times now to ask that that be corrected cents they didn't even have a legal document in February and we didn't have access to the space until mid March. It appears that they have lost a few administrative staff people and a maintenance guy since we moved in. That seems like quite a bit of turnover in two months. The sales person said how they like to do a big move-in celebration and make their new residents feel welcome. Basically the celebration was me running her down in the hallway so that she could give Mom a gift bag with a blanket, a mug, and a word search book. They seem to have several empty rooms so I think that is why they are focused on sales but then drop the ball...I would probably not recommend this facility, you can find something just as good somewhere else , like their sister community that I wish we could have gotten into.
