When mom & dad both developed memory loss / dementia at the same time, we were suddenly faced with the challenge of how to care for them. We evaluated many care facilities in the area, but felt Autumn Leaves was the only one totally devoted to memory care. Our experience with Autumn Leaves over the past year and a half has been very rewarding. While any facility can provide for the necessities, Autumn Leaves management and staff exhibit real human warmth and compassion when caring for their residents. We have visited at all times of the day and night, unannounced, and the care has always been consistently good. It is also reassuring to know that there is a nurse present 24/7 should the need arise. There is real peace of mind knowing our parents are in a clean, safe home-like environment and are being well cared for.

Autumn Leaves of South Barrington is truly a beautiful, home-like environment for my loved one. The staff is friendly and well trained. The 24 hour nursing care is wonderful. Since it is brand new it has all the bells and whistles that you would want for the ever changing world of memory care. It is spotlessly clean and the furnishings are beautiful. The food is delicious and served in a homey environment. There are daily activities for the enjoyment of the residents as well as monthly support for family members. I give it a complete 5 star rating.

From your very first steps onto the property Autumn Leaves is as lovely as it is welcoming. Their staff is warm, trained and friendly. Since it is brand new it has all the bells and whistles in this ever changing world of Memory Care. Including support care for families. The 24 hour nursing staff are wonderful and keep your informed. No matter what time of the day or night, I go the residence is spotless. The food is delicious and is served in a pleasant atmosphere. It feels like a home and not a facility. Daily activities are planned for the residents enjoyment. It truly is a lovely facility

The facility had literally opened a month before Dad was admitted. It's a beautiful setting all on one level with capacity for 45 residents. Dad was number 12 or so and we were able to select his room from those not already taken. The entire staff, from healthcare to operations is the most wonderful, caring group of providers. The family is very comfortable with Dad's care. As he is the oldest and most feeble of the residents, he is well attended to. There is a chef on duty to prepare all the meals which are healthy and very delicious. The nursing and professional staff are very dedicated and well trained to handle the memory-challenged at the facility. I give the Autumn Leaves a 5-star rating on service, care and facility environment.

They do a really good job. It's very clean and the staff is very friendly. It's a good value, it's 1100$ less than the last place I was. They are very attentive, and they have lots of activities. The food is good, I am happy here.

We made the selection based on location. The location is the most convenient of nine options that we looked at, to all of us, the closest and most convenient. Value was a consideration high on the list; the cost is the most reasonable. It had to be a memory loss facility. She was in a situation that didn't specialize in memory loss and there were gaps in her care. They seem to have plenty of activities to do all of the time, though my mother doesn't necessarily take advantage. We're still trying to resolve questions about when the family needs to get engaged, and how to get them engaged in certain activities; for instance, what family members to call; that just takes time. They've always responded to us when we raise something. For instance, my mother lives raw fruit and vegetables. Most people at senior centers do better with cooked vegetables. They have made a change for her to make more raw fruits and vegetables available to her. We think this facility is a very good choice.

They are probably the best place I've ever seen. They are fabulous. I haven't had any concerns about anything, not the food, or the activities, not anything. They are great! They are clean, and friendly, and we couldn't have found a better place.

They are doing a great job. They have really good caregivers they are really committed to giving great care. Our loved one is doing really well. We haven't had any complaints or issues with the food either. We are really happy with them.

I was lucky to find a place so close to home. It was a home away from home for my husband.

Everything is going well at Autumn Leaves. I'm satisfied with the services, care provided, friendliness of the staff, and cleanliness of the facility. So far so good. The staff is very accommodating.

I have nothing negative to say about Coventry of Mahtomedi. The facility is beautiful! The place is wonderful, and staff is very caring. I'm satisfied with everything.

Our family had experience with senior assisted living communities, but had not looked into what happens after that. Events occurred very rapidly and we found ourselves looking for a memory care facility. APFM was able to give us eight different facilities to consider. We were impressed with the memory care programs of all eight. The family developed serveral criteria to narror the list - available vacancies, access to out doors, smaller rather than large, and proximity to family. Using these criteria several family members visited the sites. While we each saw each facility differently, we quickly began to build a consensus that Autumn Leaves best met Mother's and the families needs. We affirmed our conclusions with Mother's physician. We conversed with out of town family members who were unable to visit the sites as to our findings in order to bring them into the process. Once the family had all agreed, we moved forward to transition Mother to her new home. Autumn Leaves was very cooperative in assisting us with the transition.

This was absolutely beautiful! It looks like a nice hotel with a fine dining room. It's very clean and well maintained. They have large bright apartments with lots of windows. They could offer a lot to a social person such as my loved one. Teri Bradford was very interested in finding out about my loved one as a person in order to find the right level of care. Could be a great fit at a different time.

This place is really good. Staff goes out of the way to help. Place feels like home and comfortable. Staff makes everyone feel good .

My step dad became a resident here about a month and a half ago. We have been absolutely blown away by the kindness of every single individual that works here. I honestly didn't think a place like this was even possible. The way they continually show compassion and honor to the residents and their families is better than anything we could have hoped for. Tarran (sp?) the manager sets the pace of creating a great culture, but there really has been no exception. Everyone is just great, and we are so thankful.

They've done a fantastic job here so far! It's a great place, I'd give them top marks across the board. The facility itself is clean and well kept, and the staff are all very friendly! They do a particularly great job keeping families up to date on how their loved one is doing. I've also had a chance to try the food, which I liked! We toured quite a few communities, but after seeing this one first the rest paled in comparison!

It's been great so far. The staff is friendly, and it's clean. They are responsive and attentive, and they have plenty of activities. The food is great. When we were touring, we really liked Carol the director of sales and marketing, she was very warm and sincere. The ambiance was another big factor for us. Those things helped us know that we had found the right place for us.

They have the best staff here. Ayisha is awesome and Joey, RN, is excellent at PT care.

Stacey RN is wonderful and is patient care driven. She takes the initiative to resolve any issues for her residents in a timely manner.
