When we started looking for assisted living facilities near Palm Harbor, we reviewed and visited 8-9 different ones. Many of their features were equivalent; for example, the room sizes and amenities were similar in most cases, as were the dining facilities. We settled on our #2 choice, Horizon Bay Clearwater (now Brookdale Safety Harbor) based on 2 of our criteria - closeness to our home and cost (our #1 choice was $1000/month more expensive.) We give very high marks to almost every aspect of Safety Harbor. The ONLY area that we felt was less than excellent was the way in which the different staff/departments coordinated with one another in cases where the resident needed non-standard attention. We give a bit of a pass here because of the ongoing merger with Brookdale. Also, the week we moved Dad in, the executive director was absent due to a training meeting, and the head nurse was out on a critical day in Dad's care - no one's fault, just the ball taking a funny bounce. Likewise, the less than excellent rating for the activities is more of a reflection of how they fit Dad's needs than on the quality of the activities themselves or the dedication of the staff. For example, Dad, is a dedicated walker, and it is not realistic to expect the facility to accomodate his capacity for a daily 3 mile jaunt. But his needs are also not met by chair aerobics. If Dad's future is in memory care rather than assisted living we can accept that. However, in that case we MAY have to find a facility that has more secured outdoor walking space (there are 1 or two in the area) If your loved one falls clearly within the assisted living or memory care category, you will be very happy with Brookdale Safety Harbor. Horizon Bay Clearwater has been working very well with my friend. The staff are very vigilant they make sure that he is comfortable and that his care needs are being met. The community is clean and up-kept at all times. The food is great it's tasty and very healthy the portions are very good as well. He is still having a little trouble getting adjusted to the community but overall everything has been great.

We toured this community while looking into care options for a loved one. I thought Brookdale Countryside was a very nice looking place. It had a very upscale, social feel to it. The employees I met all seemed friendly and professional. However, I don't think it was quite the right fit for what my loved one was needing at the time.

I think that they are great. it has been a very positive experience. The staff has been tremendous. The negatives are the food though. It is not up to my standards at all. As far as the care, I think that there are certain staff members that could do better about keeping the patients tended to, cleanliness-wise and that sort of thing. The big thing is the food though

We have had a very good experience with Brookdale Countryside. They worked with us very well to get our loved one moved in as quickly as possible. She was made to feel very welcome. The activities director especially has done a great job to make her feel right at home. I can't think of anything they could have done better. It has been an adjustment but they have accommodated us very well and have been extremely friendly and helpful.

It would be an okay place for your loved one to go to, it is a nice community. The food seems to be institution or hospital food like. Some of the activities offered are: art classes, games, and some additional for the residents to choose from.

I really like it here, everybody is so friendly and helpful. The food is mostly good, but once a while things there are things that I can't eat or don't like. To be expected, I guess. I like the weekly cleaning, vacuuming, etc and laundry services. The only thing I don't like is that they are charging me $137 a month for an oxygen machine. I didn't know upfront that it would be an issue. It may be resolved, however. Overall, I would recommend it.

Our first impression we were pleased with how the place looked. We also liked the friendliness of the residents and some of the staff members. We received pricing for the room, meals, transportation, electric, water and cable but we did not discuss care giving prices. Because my parents are applying for VA benefits they were required to use at least 2 of the services at the facility. I did have to meet more than once with the head of the nursing department. I also had to meet more than once with Amy in the business office to have the incentive for moving into Brookdale Clearwater deducted from my parents bill. At this time I am noticing that a lot of changes in staffing is occurring which I hope will improve the dining experience, as well as, transportation services. There are great people working there such as Rita at the front desk. She is phenomenal and makes sure that things are getting done. There are also a couple of resident aides, Ceelee, Chantelle and Yanis. Helen is in housekeeping and she is a sweetheart. The new hire for the business office, Stephen is good too. My other favourite people are Caroline, because of her positive attitude and Lorraine in the beauty shop. My Mom gets a big kick out of her server, Leonora, in the dining room. My Dad just would like his food hot. The place is well kept. My only suggestion would be that when you talk with someone that they should write it down so they can remember to get done what you are asking them to do. We walked in the garden this week and the covered area needs a really good cleaning. And I wish we did not have to get out there through the smoking area.

For months the kitchen was out of basic staples, like eggs and bacon, milk, serving cold sandwiches as dinner, serving the same 3 or 4 dishes over and over. Transportation is part of the package residents pay for, but they have not had a driver in months. Also they are terrible with the laundry. Constantly out of detergent, problems with the machines and mixing up peoples clothes / losing clothes. The hands on staff and medical director [name removed] do a very good job, but they are chronically understaffed. They are on the 3rd facility director in 12 months, and it is very inconsistent service. Communication with the families of residents is poor overall.

Community is very safe. The Activity Department is very proactive in encouraging residents to participate in different activities. The facility is clean and dining room offers a variety of meals to satisfy most people. They do need more aides to assist the amount of residents and more monitoring.

I would tell them there has been a decline over the past 3 months. Hopeful with the new ED, things will improve overall. Census has decreased, and staffing is reflecting that. Food quality is subpar which is a change since admission.
