This community was the most filthiest place that I have seen. the staff were nice the management was not. Please do not take your family or animals here. The staff that help with the residents were great with us.

Memory care is horrible. Very little was positive.

The review letter sent in November of 2015 is spot on! So much so that I thought it came from my wife. A lot of the staff is very friendly.

The administrators and staff at Brookdale South Hill are extremely rude! After jumping through all their hoops, bending over backwards to help them and doing 90% of the work to get our mother assessed for residency; the Director Scott was a no show to a prescheduled meeting, and after waiting over 45 minutes for the administrator Susan, she rudely denied our mother and made her cry in their lobby.

Not suitable- this one looks like it's 100% Medicaid. Many of the residents are on motorized wheelchairs, and look like Vietnam vets. Small, Small rooms. only one closet. Very old building. Shabby. Not good for my 91 year old mother.

I did not have a positive experience, I thought the medical care was pretty inadequate. When I would inquire about my loved one's health, they would let me know that the stats were abnormal, and when I inquired what action they were taking, they said that they thought it was faulty equipment and they didn't seem to care to do anything, without prompting from me. I found some pretty advanced wounds and they didn't think it was a concern. They didn't properly address it even after I had expressed my concerns. When we set up transportation, for a crucial appointment, they were not able to get to the appointment on time. They would take 30 minutes to help her to the restroom. While it looks like a pretty place, the medical care is atrocious. They would order prescriptions, even after she was not there, and I had to pay for them.

Short staffed Care services nit provided. I pay too much money for my love on to be there. Went to the new ED, she had no idea how to solve my concerns. I am looking to move my love one out.

My husband had 2 hematoma subdermal surgeries in 2 months...plus he has 2 artificial values and on warfarin. My husband has been in the care at avamere and according to them his INR was over 5.5 which is SEVERELY dangerous and his blood pressure was very high.. No one called me to tell me the situation. I was even there that day and every day Poor health care Do not send your love ones there Think twice

This place is horrible! I would not put your family members at this facility. My father walked out it was so bad. Not enough CNA's and nurses seem stressed and overworked. Managment didnt seem to care when told. Very, very sad facility.

My 88 years old mother was just transferred there from Salem Hospital. First they said she could not call long distance to family unless she wanted to pay long distance charges. Secondly she received some dry rice and tough piece of chicken nothing she ordered and could not even eat it was so bad. She had a fall with broken hip and shoulder, and had to go to bathroom nearly wet the bed waiting so long. Then nurse came and left her over an hour on the bedpan til her legs were numb. When she finally returned there were no supplies set up for her arrival at the room. it wasn't stocked although they knew the transfer was occurring.....that's no welcome at all. I can't imagine leaving her here for her rehabilitation for any length of time. So upset, I need to get her transferred right away.

My parent lives here. The administration cuts corners at every turn. The food has gone from decent to barely edible. You would think that for the amount of money they charge they would actually cook. Many medication errors. Understaffed and minimal training. Light housekeeping- very light. What exactly are we paying for? Rethink placing your family member here.

I had the unfortunate experience of spending 20 long days at Avamere. The food was terrible. Their idea of an egg salad sandwich is a scoop of runny egg salad between two piece of white bread. No Lettuce. No butter. My roommate was taken to the bathroom and forgotten. She was yelling to get out. I started yelling too. Took about 15 minutes to get someone's attention (seemed like a lot longer).

Just moved my mother out of this place. She had lived there for 4 years without any problems. A new corporate administrator is now in charge and the place has gone downhill. Many good staff were fired for unknown reasons, many are now quitting as well. The caregivers who are left are overworked and exhausted. Call lights can take up to an hour or more before answered. Talk to your local home health and hospice nurses for advice on good places for your loved one. Physical therapists as well. None I've spoken to would ever place a loved one here

Very depressing place -98% don't smile they all look so sad. I wouldn't send my worst enemy there. My very good friend was there she became so depressed almost immediately. As far as activities not too much going on except bingo and a few others. My friend had jewelry, money purse with all her IDs cc and cash disappear along with half her clothes and even a picture of her brother that I personally hung on the wall it was kinda big. Her linens were filthy and so were the statements of acts. They lost a check and wanted to charge her $250 and then serve her with an eviction and then take it upon themselves to take her to see these group homes (private house) where 12 people shared and 4 to a bedroom with out notifying anyone. They gave her bills with no detail as for what the charges were for and when requested still never produced. I got her out as fast as I could and she is living with me. Its nice to see her smile again. what a nightmare Dont punish your loved ones-dint send them there

Don't be fooled by the nice furnishings and the good food. A skilled nursing facility is only as good as the care givers it employs and most of the people working at the Pearl should not be taking care of anyone due to their lack of caring, incompetence and laziness. I received absolutely terrible "care" while staying here for 12 days. (A handful of RN's and CNA were excellent, but were far and few in the sea of incompetent ones.) (One nurse ripped an inch of my skin when changing a dressing, saying that ripping skin is just part of removing dressing. Another nurse failed to answer my call button for 35 minutes when I needed to go to the bathroom and only came when I called the front desk on my private phone after googling the Pearl's phone number on my laptop. I was told that in the future I should just have an accident and that they'd clean me up afterwards! When getting me ready for bed, another nurse left me uncovered and practically naked with the air conditioner blowing when her shift was over. She announced she had worked 4 minutes past the end of her shift and that the next shift would come in and cover me with a sheet and blanket and walked out the door. I lay there freezing for 25 minutes before the next shift came on and finally covered me. Aside from these major breaches in care, call buttons consistently went unanswered for 10-15 minutes (they assured me they have 18 minutes by law to answer a call button!), my pain meds were routinely delivered late (again they have 45 minutes to deliver meds before they are considered late), food was served cold, and so many other small things happened I can't even keep count. At the end of the stay, I was assured by the director of nursing that while she had reprimanded those who treated me so poorly that she stands by her staff and the exemplary qualify of care that the Pearl provides. The problem actually lies with me. They had done a poor job educating me about the actual level of care to expect and that my expectations were just way too high--they couldn't possibly ever provide me the type of care I had been expecting. Since they pretend to be a 5-star care facility, I was surprised to find out that they actually strive to provide only the state minimum mandated levels of care, and that they do. So, if you are looking for dismal level care at a premium price in a beautifully furnished institution, the Pearl is the place for you. If you actually seek excellent or even good care, run far and fast from the Pearl, since you will not find that here.

would give less than one star if possible. My mother was a client of Mountain Ridge and had terrible care. She was given the wrong medicine on two different occasions and ended up being hospitalized because of it. I live many thousands of miles away and I flew in to see what was occurring. I ended up staying with her at the facility for over a month. During that time what I saw was appaling and criminal. The staff at night would dissapear to go out with their friends. Once a client fell and bled profusely. I heard her screaming and I had to call an ambulance because the CNA on duty was out with her boyfriend! (She was NOT terminated or even reprimanded). On another occassion, a newer male patient [removed] a long-term female patient and she ended up [removed] a month later. He was [removed] and subsequently removed. But again, this happened at night and there was no one around to help the poor lady. <br />Needless to say, I removed my mother to another, more appropriate setting. With that said; the neglect I personally witnessed was nothing less than criminal. Particularly for those too infirm to complain or to get help. These people are dangerous and although the facility is nice and well cared-for; I can not say the same for the clientele. Don't risk it!
