Trouble with meals. Portions are too small and there are not enough alternative foods for diabetics. Also trash is not picked up daily as promised. Maintenance has been slow to fix things I have reported.

[Removed] was on vacation, but man [Removed] was very helpful in giving me tour and info. on lady to call for Veteran's financial help. It was a little small for my parents and their dog, but nice.

I actually took my mother out and switched facilities. The initial feeling of the place is that it is so quiet like morgue. It is not a full happening facility. I experienced medication errors and high staff turnover including administration changes. My advice would be find a better place. It is in transition and not ready for a 5 star rating

We toured Traditions Senior Living and Memory Care. The facility was dark and gloomy. The person doing the tour was adamant that my loved one would have to be in the Memory Care section without knowing anything or asking anything about my loved one.

If you would pay your help what they are worth (at least $2.00 an hour more) there would not be such a turnover. Especially bad on weekends.

My mom had a stroke and they waited 5 hours to notify me and when I got there they hadn't even called for help yet. They waited for me to get there and that's unacceptable. I would not recommend them.

Our best advice: Look elsewhere, this is not where you want to place a loved family member. The caregivers work hard, but don’t have any support or guidance from facility administration. Response time and communication is non existent. It’s basically an independent living facility masked/marketed as an assisted living facility.

If you don't care about your loved ones then send them there. Even the residents constantly complain that it's worse than prison. Food is bad. Staff is too low, overworked and underpaid.Staff tries, but so overwhelming. They look stressed. Management gives promises that are so far from truth. Ploy to get people in. It's dirty, unsanitary and reeks of urine. Run fast! 0 stars

I had an interview today with the marketing director at 11:00. When I got there she blew me off and sent another person to give me a tour which didn’t help because I had questions about Medicaid and this person was very nice but answer my questions. I wish The marketing director, [name removed], would have called and let me know she wouldn’t be available. Worst customer service ever.

My mom has a bit of dementia and we needed to find her a place to live where she would have 24-hour care. My sister and my mom both toured the place and liked it, at least on the surface. We set a move in date and was told we needed to sign an agreement which is understandable. It was not the easiest process to get them to send it to me ahead of time so I could review it, but they finally relented. It was a 15-20 page document that they expected you would sign on site during a meeting without being given the proper time to read it. Upon review I noticed there was a page of behaviors that would result in the resident being asked to leave, unfortunately, almost all are associated with dementia. When questioning the powers that be about this, I was told that a large percentage of the residents did in fact have some sort of memory deficiency. My concern was that we would pay the hefty community fee and my mom (who they met, interviewed and tested) would be tossed out for one of the noted infractions. I was told I should trust them as that was not how they conducted business. However, less than a month later, they either wanted us to remove our mother or pay for someone to watch her during the time the doors are unlocked and unwatched. Additionally, there was an altercation with her roommate (a woman my sister specifically requested she not be roomed with as it was obvious on the tour they would not be compatible) where the woman threw something at her. The staff refused to find her a different room. My suggestion to anyone considering moving their loved one here, think long and hard and truly read the agreement.

Susan was not at the facility, rather a med asst who seemed surprised we were there. I had re-scheduled with Susan for 4pm, which is when meds are being dispensed, so we were asked to wait for the med asst to finish dispensing meds. Residents seemed disoriented and weren't able to have a coherent conversation with dad. We did not stay for the tour. Pfluggerville too far from Austin.

We did not have a good experience with The Oaks at Liberty Grove during our short stay here. The girl who is at the front desk was very good, she was personable and helpful as was the girl who showed us around the facility. Everything seemed like it was ready to go, but the day before my husband was supposed to move in, there were several things that were not taken care of and I was having to run around like crazy. They were not sure how to give him his medications so I had to stay with him and administer his medications. When I called the owner to tell her about this, I had some major issues with her and the way she handled things. Also, the rooms here never got below 85 degrees so it was a sauna. They told me that they were not able to care for him, they knew he was a 2 person assist and they did not have the capacity to care for him. They moved him and they had not told me where they moved him to and no one seemed to know where he was. We had a lot of issues with this facility and I would not recommend them to others who are looking.

My mother was a resident at Autumn Wind Assisted Living for almost a year. My mother and our family were happy with the care she was receiving. On [date removed], my mother went on hospice. At that time, the community was, and had been under quarantine. I was; however, assured that when she reached end of life, our family would be able to be with her. Her attending hospice nurse was denied access into the building (unless there was a change in condition). On [date removed] the facility contacted the on-call hospice nurse and reported a change in condition. The hospice nurse notified me that my mother was not doing well. She was going to monitor her for an hour and then call me back to tell me if my mother was at end of life. An hour later she called and told me my mother was showing signs (mottling) of being close to death and I should notify family to come. I relayed to her that I was on my way to the facility. I then got a call from staff, telling me I was not allowed into the building, to be with my mother. She suggested I call back in the morning. I was beside myself, knowing that my mother needed me, knowing she was wondering why I was not there. I called the next morning and was met with push back again, then finally was told I could come be with her. I arrived the morning of [date removed] and my mother was non-responsive, and died about 30 min. later, never knowing I was there. Even hospice could not believe I was denied access to my mother. This was not what anyone would want at end of life. The next devastating event: Her apartment was paid for through the end of May. My brother and I wanted to move her items out that next weekend. I was told by [name removed] (Owner/Administrator) that she did not have the staff on the weekend to oversee a move-out and suggested a weekday. On [date removed], I spoke to [name removed] and arranged a move out on [date removed]. It would be myself and my husband clearing out the apartment. I explained, we would be sorting through her belongings and have it all moved. On Thursday, after 5 pm, [name removed] called to let me know she had something wonderful to share. She had her maintenance crew move all my mother's belongings to a dirty garage. I was without words, hurt, violated, angry and so was my entire family. When I arrived Friday morning, some items had been brought back to the apartment and other remained in the garage. My mother's pictures, and belongings were strewn on the floor all around the apartment. Her shoes that were nicely arranged were in a heap, her linens were wadded in box, and many items were missing. As I would report to [name removed] what was missing, "she seemed disgusted at the situation and said to let her know how much and she would reimburse me." After getting moved and home with my mother's belongings, there were even more items missing. This was not the way her stay at Autumn Wind had to end. [Name removed]'s actions and reactions were without compassion and respect. My mother did not have a lot, but what she did have was family and her few possessions. She would have wanted me by her side, as she struggled in those final hours. She would have wanted her possessions handled with care by her family....not rummaged through and thrown in a garage, picked over, and stolen. [Name removed] (Owner/Administrator), please give your future residents and family members dignity at end of life. Take responsibility for your inappropriate protocols, please do not blame it on your staff. Though unimportant to you, my mother's final hours were important to her and our family., and so were her belongings.

This place is terrible. They do not take care of their residents at all. They aren't assisted with bathing, they have residents walking out in the middle of the night and no one notices. The state really needs to investigate this place.
