I was not impressed with this community when mom lived there about a year ago. The director of the community was not very helpful. They would not follow the order the doctor recommended for her care. The community was brand new and beautiful. I would recommend looking at this community as things may have changed since she lived there.

My loved one is currently a resident at Oak Pointe of Carthage. The move in process was very easy and management was very accommodating. The housekeeping does a good job. The friendliness and the staff are both great. The food is okay. However, I am moving my loved one to a new facility soon. Her new facility offers activities from 10am to 8pm and will be a better fit for her. This facility is good, it's just not a good fit for my loved one.

My mother has resided here close to 2 years. We chose this facility for the promise of graduated care that we were assured would be available as she aged and her needs increased. Things went well while Mother was relatively independent, designated as “Level one”. We’ll now she’s “level 4” and the care she’s receiving is not reflective of that. Plan to have a family member present at least 16 hours a day to supplement your loved ones needs because the staff is overwhelmed, overworked, and under trained. The people passing meds are trained on the job and have minimal competence to be administrating drugs. Off shifts(nights weekends and holidays) are a real challenge. Skeletal crew and no resources to address problems that arise. This place is very high priced and not meeting expectations. The staff turnover is a problem. Always new faces. I issued a complaint with the regional corporate operational VP and was assured they were working to fix the problems. Time will tell.

It's not ideal for those who have a loved one who has dementia yet is younger than 60. They don't have a staff that is educated on specific types of dementia. Communication can be very difficult. When looking at costs, be very specific to find out what exactly they are including. The front people are very nice. But the staff who are there in the evenings are not very friendly at all. However, there is one shift manager who is really nice. The executive chef is really nice as well. My loved one does not like the food, but they try really hard. They just don't have ethnic types of foods that we're used to, but they're willing to listen to your suggestions. Because of Covid, they have been understaffed, and unfortunately many of the residents have had to endure sitting in their own waste. And you could smell it in the hallway at an overpowering level. Sometimes they would leave the garbage sitting out in the hallway for quite a while as well.

My loved one was there for a very short stay because they simply were not able to provide the level of care that he needed or handle dementia-related behaviors. I think their heart was in the right place. They were just understaffed. And I'm not sure that the staff was well versed in how to take care of people with confusion and dementia. Memory care in that facility was a big overstatement.

The executive director and director of nursing are incompetent and truly do not care what happens to your loved one they are simply there for a paycheck. Once it hits 5pm they sprint out of the building. The only reliable, caring staff of Provision Living have left, due to the lack of support from the executive director. They are unable to keep staff. Yes — the building is nice because it’s new but there is not enough housekeepers to ensure that the facility is getting cleaned properly. The turnover rate of staff is so absurd that it would make me genuinely nervous to recommend this place for anyone. I suggest looking elsewhere when searching for a place for your loved one.

Provision Living at Forest Hills is not as it appears aesthetically. Provision Living is managed by a Missouri company, and all decisions are made out of the corporate office. The in-house, management “team” is inexperienced, 9-5 workers with a corporate mentality. All seems to be done for appearance sake, and not for the good of the residents. There are so many inconsistencies, and very little, if any, follow-through. The activities are sometimes held, sometimes not, with lack of communication to the residents. They promised outside activities on the bus, but not all who sign up can fit on the bus, so they cannot go. This community is best fit for those who are independent. If your loved one needs help, do not look at this community for your loved one or yourself. As your loved ones age, and need more assistance, this is not the place for that. The management is reactive, and dont have established systems in place. They seem to be reacting to events as they occur, then they cover their tracks. The head of the nursing is an LPN, who is not competent in her position. The care staff is good, but they are overworked, and understaffed. The building is too big and the care staff has to go from one side of the building, and up and down. It is a bad configuration. Medication management is a problem. There are frequent med errors. Pills are missing, over/under medicating your loved one. There is no sense of urgency nor follow-through. There is favoritism toward certain residents, who basically run the activity times, the dining room eating groups, the outings, and who gets the most attention. The television is DirectTV, and it goes on and off frequently. If you are watching a football game etc, it can cut out for hours. The food is not good. Pork and beans, and soup and bread, and stuffed cheese hotdogs on a croissant, for example in one week, are not heart-healthy choices for the older population. Many basic food items run out- for instance, no lettuce for a week, no chicken breast, not many fresh fruits or vegatables. The wait in the dining room can be up to 45 for the food to be served. The dining room is extremely cold, as is the food many of the times. Residents are tired of complaining, and have waited so long for their food, that they are hungry. The management is the problem. They are condescending, and act as though you are a bother to them. They are trying to make a presence in the community, but are forgetting that the paying residents, who have physical and mental needs are their main income source, and responsibility. This place has only been open for going on 7-months, and you should be aware that it is not as it appears. They put on a great facade for the community. Be very thorough in your search for your loved-ones. They are great at “selling”, but what they are selling is not the reality. The residents are afraid to speak up because of fear of retaliation. Most bring things up to the staff, but nothing changes, so they keep quiet. They will kick your loved one out for any reason. One such beautiful 91-year-old person was kicked out after four-months of being at Provision Livind. It was heartbreaking. Looking for a place for your loved one is not an easy task. Even though we asked the right questions, the answers given by the Provision Living sales people were very well scripted. Please take this review with some merit, and be cautious. Your loved one’s care is of utmost importance, and tread lightly if you are thinking about moving them into Provision Living Forest Hills. Things aren’t always as they seem.

My dad fell many times with visible injuries, we were rarely contacted to report the incidents. They claim they have footage to share with families from technology in the rooms, but often said they didn't know what happened to him. A ton of staff turnover at all levels. Missing items that were purchased for him and his medical care never to be seen. Laundry not done, so we had to do it ourselves. 1 or 2 med techs for up to 14 residents in memory care and he was not given basic care that was medically instructed like cutting food or help with hygiene. My dad was taken out of the building alone with a man saying he was there to transport him to a dr. appt. We were never called to make sure he should be leaving without a family member attending. We would've never allowed that, ever and this appt didn't even exist. This just scratches the surface! My dad recently passed away and they never called to give their condolences. Unprofessional and frankly very sad after all we went through.

I am very disappointed to see a reputable organization like A Place for Mom would recommend this facility. We had my dad there for just over a year and had nothing but problems from medication errors, failure to clean the room properly (including leaving poop on the floor for over 24hrs), lying to us about sending out at urine sample, and the list goes on. They have extremely high staff turnover.. from the time my dad moved in till when we moved him out only 3 staff members are the same as the day he moved in. The current director brags in letters to the families about how she has hired 64 staff members in less than a year… how can anyone expect any quality or consistency of care with such high staff turnover!

Between my husband in Memory Care/Assisted Living and me in Independent Living, we paid Provision Living [fee removed] per month from late April through early November to live at their Crown Ridge facility in Walton, KY. During my tours in March and April, I was promised 24-hour on-site nursing in the facility, consistent supervision of my husband's taking of medications, the ability to complete on-site urinalyses for my husband as needed to check for UTIs, an on-site lab to complete such tests, assistance with my husband's cPap machine and meals that accommodated my gluten allergy. None of these promises were kept with any consistency. So, we left.

I have both, my Uncle and Aunt in this facility. Don't let the decor, big bright windows, and photo gallery fool you. My Uncle is diabetic; has never seen a dietician, and given pasta, potatoes, hamburgers with bread, and also sweet desserts! Diet pop for drinks. Yet, they don't forget to give him his insulin shots every day. Makes NO sense! I am told there is a physio therapist who comes once a week!!!! There are hardly any activities or entertainment like you find in other facilities. If you want such services you will be asked to pay more. Some of the staff members are nice, however, they don't seem to have formal training/certification in looking after such residents. They seem to be under-staffed, and this puts a strain on the staff members. Also, their Fall Prevention tech never worked for my Uncle, when he had couple of falls, and lay on the ground, till help came. His only pastime is watching TV. Do you wish to have your loved one in such a facility? Think twice!

The number of caregivers need to be increased. While visiting, there would be 15 minutes or longer when no caregiver was in the common area. When someone needed help, I, as a visitor, would help them. .[name removed] were served cold. Salad would be set on there table with no utensils, no napkins, no drink. My mom enjoyed coffee with each meal. She would not be given coffee unless she asked. Then it may or may not be given to her. In Memory Care, there is no routine except for meals. No music time, no exercise time, no activity time. If there was a craft, it was sporadic. Mom wasn't taken to the Assisted Living side for trivia or other activities she enjoys. She would sit in the dining area, common area, all day with nothing to do. The Executive Nurse was rude and would discuss my mom in front of other residents and visitors. Executive Director doesn't respond to emails for days. The trash in unit is not emptied daily. It accumulates for days which begins to stink. Need for training

I had to remove him a few days after admittance because he kept getting out into parking lot and I was asked to spend the night to keep an eye on him and then asked to hire a sitter. All this after having to furnish his room and bathroom!!!! There was no place for him to walk except a small sunny courtyard at Provision. The workers were not attentive to the bed ridden either. My experience with provision living was terrible. The nurse came to assess my mom at Porter Hills and said they could take care of her. She was there for a week and they said that they were sending her to the hospital and would not take her back because of falls which they assured me they could handle. But they didn't. The hospital sent her to Saint Ann's where she died. I would not recommend provision living to anyone. It's a beautiful place, but it's deceiving. They fed her in her room and she hardly ate. Medical care was bad. Hospital said Falls were from fluid overload. I'm still angry. When I called to talk to the marketing person, she was no longer there.

My loved one was previously a resident at Provision Living of Forest Hills and the overall experience was terrible. I was very disappointed with this facility. There was too much staff turnover. The sales person was terminated the day that my loved one moved in. The head nurse said that she was there for the long haul and then quit a short time later. The caregivers need to be trained. They mixed up pills several times, including other residents' pills being left in my loved one's room. Also, my loved one would have to wait an hour or two to get help with the bathroom. The caregivers also were not trained on using a belt for moving a patient. It was a disappointing experience.

When my mom was first placed in this facility everything was great. Then the COVID virus hit hard. My mom survived it well. She was asymptomatic. I wasn't allowed to see how she was in her room. I worked many years in similar facilities. I knew that this company was going from a 5 star rating to a 1star. My mom went "downhill after her first major fall.". I was allowed to spend special time with her during her hospital visits. The last time I was with her during hospice time, she had dry rotting teeth and broken down skin from not receiving the needed 2 hour monitoring care. Yes. There were caregivers that were excellent when they were there for my mom. I still pray for and to my mom hoping that she is at peace and that the pain has left her.
