We looked at this place when we were searching for independent living. There is a large buy-in fee when you move in that will allow you to move on to the assisted living portion when necessary, but assisted living portion looked very bad. The independent living portion looked beautiful, but the assisted living is NOT somewhere I would want my loved ones to live.

This place has gone down hill since new manangment took over about 3 years ago. All they care about is money . It’s a shame because you would really enjoy coming to this place and now it’s makes you sick to your stomach how people really are. The aides are horrible. You always see them sitting around more than working and asking anyone questions nobody has answers . The activities is horrible over in the dementia building . I never see the person working with the elderly and they looked desperately bored and it’s a shame . Not a good place to take your loved one .

My Mother was in Juniper Village from August 2014 to February 23rd 2016. She had dementia and had become some verbal. Some of the help knew how to redirect Mom and others did not. Juniper, needs to have more training for their help when it comes to dementia patients. Some of the help is very caring, others are there just for the paycheck. The week of February15th the children received a letter stating that we need to get our Mother out of Juniper Village in 30 days and find another place for her, and that we would need to provide 24 hour surveillance of Mother while she remained there. The director called both my brother and I while in church (had to leave) that Sunday morning and told us to hurry up and get the 24 hour going. With all our busy lives we did do this request until we hurried to find another place for Mom. Juniper does have a memory care, but the director did not want to put her in that part of the facility. The Director thought it was better to separate my Step Dad and my Mother instead of trying to work with the family to keep them together. It was heartbreaking the night (February 23, 2016) we had to take Mom to another place. My mother ended up falling at the place we chose to take her. She had bleeding in the brain and ended up in the trama care, then with hospice and she passed away on March 26th, 2016. We know that our Mother could have been with us longer , which hurts all of us deeply. I think this was a personal thing against the family with the Director, because we were very hands on with Mom and they do not seem to like it when we would question things. The in house doctor changed her medication without calling the family and then they wondered why Mother had changes. We questioned this and the director would not listen. Took the Doctor's side. Juniper has failed my Mother and the family. We were told from the beginning that our Mother could stay there until her end of life. They do not try to work through problems. They think it is best to get the resident out of the facility. This is why Juniper's ratings are high. They do not fool around with problem residents. They are cast aside. This has been a very sad journey for the family, but we know that Mom is having a wonderful time in heaven. God is blessing you now, Mom!! To the staff at Juniper that cared, Thank you so much!!

I would never recommend this place to anyone else. The promises they made have not been kept. I don't feel like they are taking proper care of her and some of the people that work there have attitudes when we ask simple questions. I'm so unhappy.

We had enough and moved to a lovely new facility. Between the missed medications doses, violent behavior of residents, screaming at night of residents, and issues with cleanliness and hygine, I had concerns about my mothers safety to the extent I contacted HHs and the ombudsman. In my opinion, until the management team can envision the facility free of these issues, nothing will improve. Im hopeful the new company will review the old grevience complaints and make the appropriate staffing and policy changes necessary to correct these issues.

1) Skilled Nursing unit, dirty, mice, needs a good paint job and it's extremely old. But the worst part is the doctor is a disgrace, he stares at the floor, if he comes in the room to visit the patient and family is in there he walks out, a lot of the clients call him [Name Removed] cuz he doesn't talk to them. The nurse manager rolls her eyes at you, doesn't dress professionally as a nurse manager, and the care is very neglectful. Here's a few good nurses AIDES and that's it. The first day my family member was admitted there, the AIDES started talking to us about the nurse manager how fake she is. I thought oh that's terrible that they do that, but boy they hit the nail on the head. They're never allow a family member to go to the skilled Nursing facility there you will hate yourself for it as I hate myself for letting my family member go there. And you try to talk to the administration, and all it is to them is a business. Be smart and look elsewhere.

Food is pathetic, served burnt and late (I have pictures). People go to bed hungry. I have numerous complaints from residents. The maintenance manager has filled in for kitchen staff that doesn't show up. High employee turnover is a direct reflection of incompetent management. They will gouge residents for money at every chance. Just months ago they allowed a dementia patient to walk off the property, gone for hours, helicopter deployed. STAY AWAY!

I was not impressed at all. The building was filthy, residents were unkept, courtyards are not appealing, and the dining rooms are nasty! I spoke with another family member while there and they said the food needs more nutritional value which is so important for those with dementia. They sit the residents in front of empty tables or the TV. This was my second time visiting this year and everyone I met isn’t there anymore. I really liked the sales girl I spoke to and when I called her she said she parted ways with the building after 5 years there. I cannot see my mom living there and being cared for.

If I could give this dump zero stars, I would. I wouldn’t refer my worst enemy to live here. Do you really want your loved one living at a facility that almost got shut down by the department of health? Not to mention, Juniper cares about your money. Once you’re on Medicaid, forget it, they’re kicking you out because you’re no longer beneficial to them. Just look at half of the reviews, most of them are about pie and one is from the executive director herself to boost ratings because even she knows this place is trash. One word of advice- DON’T. I moved my loved one out of there as soon as I saw how terrible this place is. Most other family members feel the same. It was the best decision I’ve ever made.

Food is mediocre. Bad or non existent communication between aides and nurse. We moved out. When asked for corporates phone number...I was told no one at Juniper had it....looked it up on internet. Gave them a call. They were very receptive to my complaints and concerns. No hot water for over 30 days. Told to shower in another room...

A couple of years ago my mom had to move my Oma (grandma) from Florida up to Monroeville. She was diagnosed with dementia and my mom wanted to find a facility that catered to her needs. One day my mother visited my Oma and observed bruises and swelling on her that she told her she sustained from a fall. When my mother questioned staff regarding this, she was told that they had no record of her falling. So they are charging you thousands.of dollars a month for a secure floor that specializes in dementia but they don’t even realize that a resident has fallen and sustained injuries. When my mother started voicing her concerns about my Oma, [name removed], the administrator, was extremely defensive. Then all of a sudden [name removed] starts to say that my Oma has a specific type of dementia (mind you- not documented by her doctor, just per non-doctor [name removed]) that they are not equipped to deal with and that my Oma is a harm to herself and others and that my mom has 30 days to find her a new place to live.
