A Place for Mom has scored Bay Harbor Assisted Living with 8.5 out of 10 using our proprietary review score based on 33 reviews in the last 2 years. Over all time, Bay Harbor Assisted Living has 78 reviews with an average "overall experience" of 3.9 out of 5.
The lack of staffing has been the biggest issue. There is a major lack of fresh vegetables on the menu as well. There is no correspondence back to the families either. Overall there are just a lot of improvements that The Koselig House could make to improve the experience.

Sometimes when you push the button to leave the facility it can take awhile for someone to come and get to you especially over the weekends.

The communication hasn't been the best when it comes to trying to get in contact with my loved one through the staff. The staff don't follow up on a consistent basis with family with how our loved one is doing and their medications.

My father is living here. Some staff pay better attention than others. Some common sense could be applied like sending patient out in PJ's for appointment rather than dressing up in street clothes. Upper management are not responsive or communication is lax. If I cannot get answers or proper communication it makes me wonder how my father may be taken care of.

Disliked the religious focus, but that's what it's like in Wisconsin.

Bay Harbor Senior Living Facility Review While this facility has many kind and dedicated individuals on staff, there are notable concerns that impacted our experience. Some hourly staff members lack sufficient training and expertise, which raises concerns about the quality and consistency of care. The fees are extraordinarily high considering the limited support provided under the label of "assisted living”. Our experience didn’t even include regular nursing assessments or supervision. The building itself is outdated—windows leak, and due to an old boiler-based heating system, the in-room thermostat does not regulate the temperature. We had to run the air conditioner in February to regulate the temperature. Housekeeping services are subpar, and cleanliness often falls short of acceptable standards. Additionally, the facility continues to charge full rent and "assisted living" service fees even when a resident is hospitalized, which feels unreasonable.

Stay away from Thee Koselig House if you care about your loved one! Things started out great last August, but after the Director [name removed] left, things went downhill in a hurry. We moved our family member out months ago, due to the lousy care, constant turnover of staff, and terrible meals (which was often cereal for breakfast, sandwiches for lunch, and Papa John's pizza for dinner....and we were paying [fee removed] a month for that? Ridiculous! Poorly managed, poorly trained staff, terrible facility!

2 1/2 years ago we were so happy to have a family member there. It was a 5 star facility. Now 2 star facility. Currently looking for a new place.. Administration is never around. Short staffed majority of the time. There are care staff that go above and beyond. But they can only do so much. In the last year kitchen food and services has been terrible. Don’t know what happened. Used to be very good. Families have been very upset. Not getting services that they have paid for. Residents wait to long for call lights to be answered. Very sad

Not much good I can say, some staff are great. Others don't deserve to be around people in general. Management...well not much to say about people that disregard good employees and stand by worthless employees. God forbid having a loved one live there, then raising the rent and blaming it on having to pay staff more.

Disorganized upper management. Poor quality of food and unhealthy. Short staffed. Many empty rooms now, since rates raised 2 times in 1 year. Big turnover with staff. Here today is gone tomorrow. Front doors are left unlocked at night. Too many problems to list.

Do not put a loved one in this facility. There is usually an average of 2 staff members on a shift covering 20 residents. There is no house keeping or laundry staff. Those who work do their best with what they have. Family members need to visit more often and at off times to see the real workings of the facility.

The owners of this corporation only care about putting money in their pocket, and not the Residents and staff. The owner has eliminated positions including the activity aides, dietary aide, and receptionists and now expect the caretakers to handle these duties in addition to lowering the starting salary! They are also filling up the beds with high needs residents, with less staff. Management is hardly ever there and there is no oversight, except for the owner who claims to watch over like a big brother on the cameras. This place is going down fast! Look around- there are much better places out there!

Not impressed with this facility! Cleanliness and overall structure layout was not pleasing to the eye. Bedroom had a bed laying on the floor, not even properly assembled! Numerous carpet stains throughout the entire facility. Bathroom we visited had a toilet that continued to run. Upon entry into facility staff made no attempt to greet us. Two thumbs down~!!!

My sister Pam Pasowicz didn’t mention that the when my mother fell from her wheelchair she broke the 1st cervical vertebrae, called C-1 or the atlas. It’s the 1st vertebrae under the skull. WHERE WAS THE STAFF???? When she hit her head in the nightstand, I was visiting my mom about a week before Easter. I had gotten some Popeye’s chicken for her (her favorite) when I noticed a large bump on her forehead. I asked her what happened and she told me. I stopped a staff member and questioned her about the bump. She had no idea mom had fallen out of bed and hit the nightstand w/her head. I asked if she had been taken to get checked out, she went and checked her chart and no she hadn’t. I then asked why my sister and I weren’t called. All she could do is shrug her shoulders. My mom DIED EXACTLY 4 months to the day she broke her neck.

I AM AFRAID I DO NOT HAVE A GOOD REVIEW OF THIS PLACE. THEY DID NOT HAVE THE MEMORY CARE THAT WAS NEEDED FOR MY MOM. WHEN MOM FELL OUT OF BED & BUMPED HER HEAD, THEY DID NOT CALL MY SISTER OR MYSELF OR TAKE HER TO URGENT CARE. WHEN SHE FELL OUT OF HER WHEELCHAIR, IT WAS THE HOSPITAL EMERGENCY ROOM THAT CALLED TO LET ME KNOW THAT SHE WAS IN THE HOSPITAL. SHE WAS INPATIENT FOR OVER A WEEK WITH A HUGE KNOT ON HER FOREHEAD. WHEN IT CAME TO MEALS, PATIENTS WERE NOT GIVEN ANY CHOICES & THE AMOUNTS WERE LIMITED. MY MOTHER DID NOT EAT FISH & THAT IS ALL THAT WAS OFFERED AT AN EVENING MEAL. SHE HAD HER NOOK STOLEN.
