Bay Harbor Assisted Living

So far we like The Koselig House, we are really enjoying the peacefulness of the place and just the overall friendliness of the staff members. The entire facility is extremely clean, but sometimes the individual house keeping in our loved ones room could use some help. We like that the community locks from the inside so the residents are able to wander the entire facility safetly. Based on the experience we have had so far, we would recommend them to other families.

I am very satisfied with and grateful for the care my mother receives at The Meadows of Fall River. The facility is well-maintained. There are loads of activities and events offered. The nurse and director are very responsive to questions. The staff work exceptionally hard. The price is competitive with similar facilities. Highly recommend!

My mother in law has been a resident of the Meadow’s for over [dates removed]. She began her stay in the assisted living and has transitioned to the memory care unit. We feel like she is happy and healthy, which is our main goal. Staff has been very attentive and courteous in all aspects of her care! ??

The entire staff is professional, friendly and helpful with any questions that may come up. We certainly find a home type feeling throughout the community.

Our mother was a tenant at the meadows. If you want a small town feel, excellent care, a costant variety of activities at a reasonable price, choose the meadows. They took such good care of mom. She eventually needed advanced care and again the meadows took such good care of her. She was reluctant at first to leave her home and move there, but once there a week or so, her comment to me was, "why didn't I move here sooner?" Thanks to all the staff at the meadows

My mom has lived at the Meadows for 10 years and my dad was there for 6 years until his death in 2016. My family is very appreciative of the home The Meadows has created for my parents. As my mom ages (93), and she requires more assistance, the staff is always kind, compassionate and helpful. My siblings and I are truly grateful for all they do. We would highly recommend The Meadows to anyone looking for help for that next phase of life.

As a retired community nurse case manager I’ve seen countless facilities. When I needed a place for my sister I wanted it in Columbia County. The Meadows was a bit farther from my home than I wanted but when I walked in I knew this was the place I had been looking for. The staff is amazing, the meals are wonderful, it’s clean, the activity staff goes above and beyond. They have family gatherings. I can’t say enough-I highly recommend The Meadows. She’s been there since [date removed] and we both agree it was the best decision we ever made.

This community is so beautiful, the staff care about the residents and over all the cleanliness is good, they have housekeeping staff and even a activities coordinator, they really have it all and am comfortable sending a loved one here.

The staff is still trying to do activities through covid and are taking them outside to get fresh air. My dad said the food is good and well rounded. It's also a newer facility, so it is more modern and cleaner. The rooms are very nice also I would recommend this facility.

Like this place has it all, from assisted living to Hospisc care. the nursing staff is very nice. Met and talked to several residents and they love it here. Want to take Mom to see the facility. .

This new facility is an open concept...so spacious...basically out in the country from all the others we have checked out. They had some start up pains but that is fixed and we think it is a VERY good place. They changed activities director so we expect activities to be much better. I am there for part of a day every week and everyone is nice and the place is clean. I would recommend this place to everyone.

I was very apprehensive about moving my loved ones, Mom and sister, into any assisted living facility. I didn't know much about the process and that's why I contacted A Place for Mom. With their help, I was steered toward Liberty Village in Tomah. I can't tell you how grateful I am that I moved them there. I looked at other places, but decided on Liberty Village. There are individual apartments for the residents that are efficient, well maintained, bright and cheery. My Mother and sister enjoy a 2 bedroom apartment where they were able to move in all their own furniture and belongings. I have peace of mind knowing that no matter what may occur, someone is also there to help them out and take care of them. The staff is absolutely wonderful, caring and loving. The food that is prepared for the residents is healthy and really good. If I could give more than a 5 star rating, I would. And best of all, Mom and Sis love the place.

My mother stayed at Wellhaven her last two years. It's a warm, caring environment where the staff puts residents first, providing support without being intrusive or pushy. They try to help seniors maintain as much independence as possible. The atmosphere among the residents is quite social. Mom arrived there from out-of-state, without knowing anyone but us in the area, but quickly made several good friends among the other residents and staff. When it grew apparent that the end was coming, they worked miracles to keep her in her apartment with her beloved cat. I can't recommend Wellhaven highly enough.

Liberty is a nice community for people who are more independent, as the personal living area for residents is more spacious than some other places. The staff is helpful and accommodating, and the community seems well kept and tidy. I haven’t had a meal there yet, but my mother says the food is average tasting. The portion sizes are good enough for her, and the meals are nutritious. They do have lots of activities like Bingo, which my mother loves, crafts, music, and others. We liked Liberty the best out of the other places we toured, and we’d recommend the community to others.

It's very clean and the staff is very friendly. Response time when a resident pushes their button could be improved. The meat can be dry, and some of the prepared meat is very salty. They have a new food service provider and they set the menu now, people aren't thrilled about that. They have a real chef, but they don't have him cook from scratch, or set the menu. We liked the level of care and the staff that they had here, that's why we chose them.

They do a great job at Wellhaven! It's been a difficult transition for my parents, but the staff are friendly and very caring. They're really doing their best to help my parents get settled and involved in the community. The facility itself is well kept, clean, and a very nice looking place!

I was very impressed with everything. The director was very friendly and very involve with her people. She is really very good and also her staff. They really are for the residents. Everything was perfect I give them an A+

We had a situation that occurred that I felt could have been handled better which is why I responded the way I did. I spoke to the administrator and we talked it through. It was a unique situation and although it probably won't happen again it gives me concern. I don't want to comment the details

The staff was very helpful and really cared about the residents. One item they could work on is room odors. My step father has incontinence issues and his room smelled of urine quite often. Granted some of this is health related but it was an ongoing concern.
