Avalon Memory Care

My mother does not have memory care issues, and they assured me that there were others there as well that did not have memory care issues, that that was just the name of the place but that they had other such residents that she could interact with. That was not the case at all. The care services for my mom did not go good at all. My mom was supposed to be cleaned when she was not and had urine soaked sheets to sleep with, etc. The people there seem friendly and it is a very nice facility, but the quality of the care and the fact that we were misled on what to expect made it a very negative experience. I could not recommend Avalon Memory Care.

I was told this was the best place, frankly I wouldn't put a dog here. Even if it isn't dirty, it looks as though it is. It's very run down, the furniture was old and worn out. Even the lady who is the main caregiver didn't appear clean. It didn't have a very crisp, clean, neat appearance.

No - the quality of care might have been good, but she wouldn't have any privacy at all. She was at the stage where her vanity and privacy were key to her maintaining some of her independence. They told me that she would have to take the lift seat on and off when she used the restroom, and I knew that wasn't going to happen. A lot of the patients were on hospice and it was too small. She wouldn't have felt like she had her little place in the world.

I wouldn't even consider it. They had people sitting around in the lobby in their night clothes, in wheelchairs and it was just not good. There was an odor to it and it was depressing so I would not even consider it. I didn't get past the lobby.

The residents here were all in one big room, they were not separated. My loved one was much higher functioning and they only provided activities for one level of memory care, she went downhill quickly here so we moved her out.

Rude, unfriendly and bullying from the director.

My mother was at this facility from [date removed] through mid-[date removed] The level of care, cleanliness and overall compassion severely declined. Mom would often be in soiled diapers at noon, still in bed when I would come visit. Her wet sheets from the night before would often be covered up with a blanket and remain soiled, causing a stench in the room. Gnats would fly around her trash can in the bathroom. Feces would remain on the floor in the shower for up to a week! This is not an exaggeration and I am SOOO happy I moved her. She is now getting the love and care she needs and my stomach hurts to even think about her still being at Avalon McKinney Ranch. When we left, a new director was taking over. Said she would turn it around. From what I hear, she’s no longer there. This place went downhill in a very bad way the last year we were there. The staff would even smoke weed in the office and bring their children to work. Praise God we got OUT. Skip this place and KEEP LOOKING!

I would rate everything as a 1. They are extremely understaffed. The place was not very clean. The staff was not friendly. They all hated their job. The quality of care was not great. The food was awful. They had zero activities.

I would tell others to make sure they know up front that if their person's health deteriorates they will not be accommodated and will have to leave. Know the medications and get a copy regularly so that if they have to go to the hospital, you have confidence that the list sent is accurate. Know they can move you without notice. Keep a list of all furnishings; clothes, bedding, furnishings. Get pictures of wounds and damage to furnishings. Know your rights with regard to payments. If they tell you to leave, you don't have to if you have already paid for the month.

Was so glad to get my mother out of Avalon Memory Care. They washed all the residents clothes together and my mother never got her own clothes back. They would dress her each day in someone else's clothes and socks even though we had all her clothes clearly marked. Another problem was the staff played videos and music in the common area that were often entirely inappropriate for elderly people. They played it so loud that you couldn't carry on a conversation; I would have to take my mother to her room to talk to her. The common area is in the same room as the dining area so you couldn't get away from it. One resident would go around spreading feces everywhere and they would say there's nothing they could do about it because she had a right to live there too! For the price you pay, it's disgraceful.

Avalon-Garland charged me $[removed] upfront for the full month of May, and then pro-rata charged me for April in the sum of $[removed]. They ACH debited my checking account for both sums. My husband's first day of residency was [date removed]; arriving approximately Noon. They are refusing to refund me any sums even though THEY terminated the agreement (per my elder law attorney) by sending him out. They have also attempted to bill me an additional $[removed], they say as the balance of a 30 day notice period. I was assured they would take him back even though the Executive Director [name removed] said in her text to me: "we won't take him back". The Admissions Director [name removed] said "she could have worded that better" and assured me usually once they go through medication management, they are able to come back. It became clear, when the ED [name removed] NEVER EVER called to check on my husband during his 3 week stay in medication management, that keeping him under her "care" would be a big mistake. Then, Admissions Director [name removed] said my husband could come back "IF they deemed he was stable," and that HE would be the one to make that decision. They were keeping me hanging. I filed a formal complaint with Avalon, and one of the owners who responded, [name removed] There were also issues with a controlled substance medication count sheet sent to me by [name removed]; which I requested after they sent my husband out. (I also requested a copy of his MAR, which they assert was given to my sister, but it was not. And they are refusing to give me another copy. [name removed] told the Ombudsman he had "no intention of releasing any further documents to me"). [name removed] recorded dosing my husband two 1.0 mgs of Xanax when he wasn't even a resident there!! Her records also do not account for a partial burst pack of the 1.0 mg Xanax that was given to them when he moved from [removed] to Avalon. Her record also lists dosings on days before her record says she received the meds. And one page of the record doesn't even have the correct first name of my husband on it. The count sheet looks totally "made up" after the fact. I filed a complaint with THHS on [date removed] and the investigator has advised me that the facility was given 2 citations. They must do a Plan of Correction and submit it to the state for approval. I am awaiting the detailed reports from THHS. I also was advised in my Open Records Request that an "incident investigation" was also filed by someone else against the facility, and I am awaiting that detailed report as well. They actually want to charge me $[removed] for my husband's 7 full days and 2-half days stay when they were the ones who said "we can't handle him" and sent him out?! It is robbery! My formal complaint with Avalon remains unresolved and owner [name removed] has yet to get back to me. I was led to Avalon by one of the local placement specialists because it was a stand-alone, locked down facility that was "known for behaviors". He told me it was probably the best place for my husband, for the price. They quoted me a "special pricing" range and told me my husband would be at the top of the range because he would require a lot of diversion and attention. But from the get-go, they complained about every behavior he had. I was shocked that they were so poorly trained to deal with a severe dementia patient and that they did not have more common sense in running the facility. I am more than happy to elaborate for anyone that wants to chat.

There is no medical training. I could not always reach nurse. My loved one got a severe skin infection from not being kept clean. The food served lacked calories. Based off of our experience, I could not recommend Avalon Memory Care Tuckerton.

It was a horrific experience and very expensive. I would not recommend them. They caused my loved ones conditions to be worse because of the lack of care. The interaction has been a nightmare. The facility looks great on the outside but they don't keep up the level of cleanliness..

My experience with Avalon Memory Care Fort Worth was not good. They assured me they were capable of caring for my husband who has dementia and they were not. I would not recommend them to anyone. My husband was in their care for less than 5 days and they kept over [removed] with no refund.

Avalon Memory care is a clean community we do not have any issues there and every time we come to visit everyone is so nice - the friendliness is superb. Our loved one absolutely hates the meals, a lot of it is casserole based so there are no really like fresh vegetables, salads or fruits. A lot of the residents are pretty far into their dementia so we do not really see many activities going on. The value for the money is right on point and right in the middle comparing to other communities in the area. For us we would not recommend, but the house mom is great, she is great with communication and we are comfortable with our loved one being there and knowing they are safe.

I would not recommend the community to others for their loved ones. My loved one wasn't taken care of during the time my loved one was here. The staff is not trained to handle medical needs of the resident. They force residents to walk to the restroom, when they can barely walk. The Director was not helpful in getting the paperwork to the places I needed for my loved one. I found out from other employees that no faxes had been sent, even though the director said she had sent the paperwork to the doctor's offices.

Everything about Avalon was depressing. The presentation was not the best, it was not very clean, and the whole environment and atmosphere itself was just sad. We were not impressed.

Facility is not clean. Patients do not appear to be well cared for.

My tour here was horrible. When I arrived, there were no staff around, and one of the residents opened the door. That’s how bad it was. It could have been just one of those days, but there’s supposed to be two people taking care of the residents. They were there somewhere way in the back. I would not want my loved one to be answering the door like that.

I was in love with this place until i went to see my mother an the only person i could trust to make sure my mother was well taken care of left the building due to POOR MANAGEMENT. this young lady made sure that my mother had everything she would need to live a enjoyable life there. she also made sure my mothers room well very clean so that she didn't get sick [name removed] was a great caregiver to my mother an my family wishes her the best. my mother really misses this young lady an it sading my soul to know that management in this building allows great workers to leave because they want to keep people who doesn't care about there jobs. i can say that she really cared about her job an my mother an i just wish they get management under control before they lose more great workers
