My mom was there for rehab. She actually stayed there a month for rehab and then two months after that. We didn't like it at all. The communication was horrible. They told us one thing and never followed through with what they told us. They never could get the billing right and their dining room needed help. It was a beautiful, brand new facility. We thought that we would really like it there but since they are so new they didn't have their systems in place. After 3 months there it didn't seem like it was going to get any better and we weren't willing to let them keep experimenting with their processes while my mom was living there. They even problems with the rehab care. Seeing as she just came out of the hospital she was in pretty bad shape so we really wanted great care and it just wasn't happening. All her basic needs were met but it was like pulling teeth to get them to communicate with us. The good thing there was mom's doctor there. We loved the doctor. That was the only thing that we really liked there other than it being nice and new.

Before covid, I visited several times, after covid, there were no activities, and he was in his room a lot and there wasn't much to do. Unfortunately, my loved one got covid and passed shortly after. before covid, I would have recommended this community but right I don't think I can.

IT WOULD BE EASY TO GIVE THIS FACILITY A 5 STAR RATING IF WERE NOT FOR THEIR DIETARY DEPARTMENT. MY MEALS ARE RARELY CORRECT ON FIRST PASS. WRONG MEAL SELECTION DELIVERED, NOT ALL FOOD ITEMS DELIVERED (NO SALAD DRESSING FOR SALAD, FRESH FRUIT = CANNED MANDARIN ORANGES ETC) IF THIS WERE ONLY A SINGLE OCCURRENCE, IT MIGHT NOT STING SO BAD; HOWEVER, ON NUMEROUS OCCASIONS IT HAS OCCURRED WITH E-V-E-R-Y MEAL OF THE DAY. THE LACK OF QUALITY CONTROL IN THE DIETARY DEPARTMENT, IT IS ACCEPTED AS A NATURAL OCCURRENCE. OTHER STAFF REMARK THAT THE POOR DIETARY DEPARTMENT SERVICE IS NOT UNUSUAL. THERE ARE TOO MANY GOOD EMPLOYEES IN THIS FACILITY THAT CARE ABOUT THE RESIDENTS AND THE QUALITY OF CARE SERVICE THEY PROVIDE THE RESIDENTS. IT IS MY OPINION (AND EXPERIENCE) THE DIETARY DEPARTMENT DOES NOT DEMONSTRATE A QUALITY-MINDED ATTITUDE.

My biggest complaint would be cleanliness. Once you move in, the rooms are not kept up as well as they should be.

My mother went there 12/1/17 for rehab from kyphoplasty. The rehab staff was awesome! CNA's , especially [name removed], were very attentive. Unfortunately the nursing staff on unit 400 was subpar. We felt we were totally out of the loop even though we were there everyday and asked questions. Rehab got my mother on her feet but nursing failed her. She started experiencing pulmonary edema and coughed constantly. We questioned this daily and were told the nurse practioner had looked at her. I asked to speak to the nurse practioner more than once but only a nurse called me. Finally on 1/5/2018 I received a call from the on duty nurse at 4:30 am. She informed mom's oxygen levels were low and she wanted to get an X-ray ordered. I told her my mom had an X-ray from three days prior and it was " inconclusive." I told her to call an ambulance to take her to the ER. When she got to ER she was put on a C pap machine to to help her breathe. This was at 6:00am. At 8:00 am I received a call from the customer relations specialist at River Pointe who wanted to know if we wanted them to hold her room for $307 a day. We already knew she was not going back so the answer was a resounding no. The person who called did not even ask how my mom was!

Staff quite scarce. Place clean. All residents dressed. No structured activity on a Thursday mid afternoon.

My mom and dad were at Waunakee Manor for a number of years and for the most part got good care even when they were short-staffed, which seemed to be most of the time. The social work staff was excellent and the place was kept exceptionally clean both inside and out. However towards the end of my mom's life my mom was the victim of theft as she had her Mother's ring stolen one month and on her last day alive had her wedding ring ripped off her finger. This was witnessed by two staff members. I got their statements from Susan O., an administrator. I told her I was taking the statements to the police and she told me she didn't want to get her staff involved. Well the police said they had to go interview the staff and the staff must have been coached as they gave"I don't want to get involved" answers. Ring was never recovered. My mom's hand was very bruised which the administrator to pictures of but since my mom died the next day the police wouldn't do anything about this. The Waunakee police force made the Mayberry Police look competent. This is the 2nd time I posted this review. They must have took the first one down.

Mother admitted for rehab and although most of the staff that cared for her were kind and gave excellent care, I couldn't say much for the business part of the establishment. Trying to get my Mother discharged before the 20 day allotment by Medicare was a nightmare. No one knew any answers to questions we asked, and it seemed the objective of this place was to keep the patient as long as possible to "milk" Medicare. One should be aware that often" Directors" are in need of some courtesy and to remember "WE" the tax payer are paying their wages!!!! I would never recommend this facility due to their lack of good administration.

We had an awful experience here. My father was transferred to Harsha Behavioral Center because they felt he needed his meds adjusted and he was having some behavioral issues. When he returned we noticed that his care on the level 3 was not what we were lead to believe he would be receiving. I will say that some of the staff were very good with my dad but still, he fell twice, his daily care needs were not taken care of and even with Hospice coming in for him he still did not get the care he needed. In the end, we felt for his safety and well being that we would bring our father home to live with my brother where he passed away peacefully a few days ago. I would never recommend this community and I am concerned as to what type of training the staff will be getting when they open the new dementia center there.

My sister was there for 20 days. The first week was good, week two we had to ask for everything, for them to give her a shower, give ice, clean her room. For the most part the day and afternoon shift was nice, but one particular nurse every time I walked in there, she was sitting at the desk texting on her phone. Patient lights were going off and she wouldn't budge. Therapy people were really nice, but after they left early afternoon, the nurses did not get my family member up to do anything. They only shower the patients twice a week. Who does that? It is always a problem it seems with sub-acute rehabilitation centers, my advice, if your love one can do at home therapy or care, do it. We will never go this route again.

My loved one had been staying at this facility, but we decided to move her out once she started having hygiene issues. One of the services she was supposed to be receiving was bathing assistance, but if my loved one said she didn't want a bath they would not give her one, and she was not in a state to be making that decision. After we moved her we also discovered she had a few other issues that could have been easily treated that went undiagnosed here.

My loved one was previously a resident at The Willows at Springhurst and the experience was not good. I would not recommend this facility to others.

I am often angry about the care that my mother-in-law receives at Forest Springs Health Campus. Some of the more egregious offenses have included the inability for anyone on staff to ensure that she received the latest Covid booster shot. It took me four months of advocating for her in order for her to receive the latest booster, in spite of numerous Covid cases among employees and residents. Only when I told the administration that I had contacted the health department did they follow through with multiple requests from me, from my mother-in-law, and from her husband. She is supposed to be bathed twice weekly. This does not occur. She is sometimes only bathed once per week and only after multiple requests. I could go on.

Only if you are on Medicaid should you go there. Full pay price is outrageous and not announced going in considering the care. At nearly $10,000 a month her care should have been extensive...someone to make sure her food was always correct, someone to sit with her and talk, feed her, make sure her tv was working, briefly read to her....just a bit of that. One of us was there every day so we know care was lacking. If you have the money , get private care. We did! So much better.

As a family member of a current resident I have spoken with the DON and ED about unsafe and inadequate nursing care without resolution. Calling CMS and the state board of nursing to report unsafe, negligent resident care.

This is one of the most poorly managed facilities you’ll encounter. Errors in administrations amount to illegal financial mismanagement; errors in medication and unsafe set up in terms of no medic alert pendants; residents left alone in showering. Looks good and kept clean but you should take a pass and keep your loved one safe elsewhere.

My aunt was at this community for her rehab services. Unfortunately they did not provide the level of care I was comfortable with. I feel they were under staffed. There is room for improvement in this community overall.

The staff were nice and they did keep the community clean. We did not have a good experience with this community. My loved one started to wonder and they called us on a Friday afternoon and told us they had to be out by Monday. We did talk to the director and found out they are not allowed to do that as they are supposed to give a thirty day notice. They did not seem to be able to care for him once he started to wonder. I would recommend this community for a more active resident.

Confusing layout, even for someone with all their faculties. Mom would get lost here

Nice facility but rather pricey with minimal move-in specials. Only had 1BR available.
