virtually everyone was competent and professional, the facility was beautiful and clean, the food was extraordinarily good for a facility, they had multiple activities every day... They also have polished marble floors in the bathroom... even in memory care. In addition, there were no covers or precautions to prevent residents from turning the water on by themselves. After a month of improvement and doing well, my husband fell 4 times in 9 days. His last fall, he never recovered from, and after a hospital stay that required a second brain surgery he came home under hospice care, never walked again and passed four months later. I complained about the floor multiple times and nothing was done, the staff even complained when I brought in a non-slip rug and often removed it... I visited several times before moving him there. The residents were clean and engaged. I thought I was doing what was best for him. If someone didn't have mobility or memory/fall risk issues it may be fine. However; I still believe the bathroom floors are a critical design flaw and safety issue for anyone. The staff knew it was an issue, I was there one evening as they mopped the floor and was warned to be careful and not go into the bathroom because it was slippery... and I am able bodied. His room had premium vinyl flooring... only the bathroom was marble.

This place has been a disaster for my parents since we moved them in 3 months ago. There is no food service director, no memory care director, and the executive director is a constant turn over. The place was built to look nice, however, everything seems broken. No attention to detail. No one seems to care about how the residents are. I have not heard one positive experience from any of the assisted living or independent living residents. Their words: "I've just given up".

My mother was there for two months. She hated it there. I hated it as well. People sitting in wheelchairs and urinating and no one noticing it. A woman walking down the hall after having soiled herself, crying and no one noticed. One of the servers at meals practically threw the plates on the table. No respect at all for the patients. We moved mom out of there. She lived with family until she passed away last [date removed]. She was happy. For the price of Beacon, I found it unacceptable. No telling how mom was treated when I wasn't there. I do not recommend.

We did not have a good experience with the Colonnade at Carrollwood. My father's admittance into the facility hinged on satisfactory healing of some pressure wounds he had developed on his heels during the course of his rehab stay. In spite of the fact that the Colonnade's representative came to his room at the rehab, placed her own two eyes on the wounds, photographed them and sent them to the corporate office for review, and then told us that he was qualified to be admitted into the Colonnade, they kicked us out 3 days into his stay there because their nurse on staff decided that the wounds were, in fact, stage four and therefore in violation of Florida law for that facility. We literally had to vacate the premises within an hour of receiving this news. It was demoralizing, frustrating, and completely inexcusable, in my opinion. A real money grab that took advantage of a family's desire to provide their loved one with more comfortable accommodations. I would not recommend this facility to anyone. To add insult to injury, the staff nurse, who made this startling assessment after having ripped the bandages off of his heels, left the injured heels dangling off the edge of his chair without any new dressings. They sat there, exposed to air and elements, for several minutes, if not an hour, while people in the facility scrambled to find something to cover them with after we complained. I think they might've sent him off to the hospital without so much as a Band-Aid had we not spoken up and demanded that someone do something to cover the wounds. Just egregious. I stand by my comments about the management, but I do think that the staff and the facility itself are probably better than our experience could reflect.

The first night he slid out of his chair. I rang the bell and no one came. I went to the desk there were four women looking at the monitor flashing Emergency! I told them he was on the floor . No one moved; they told me to call the fire department. He was 145 pounds so it wasn't that he was heavy. The firemen even called the director saying what was wrong with their people. Next morning (I had stayed the night), at 9:30 we woke up. No one came to his room to care for him or give his insulin and breakfast. I rang the bell this time someone came. I reminded her that he needed care. She said someone called in and she was the only care giver and he would have to wait until she did seven other people. I called an ambulance and got him out of there; I could not leave him. Hospital checked him and said he could go back. Told the nurse what happened and he was not going back there. The nurse was lovely and understood She found him another rehab. He stayed there until there from last February to his passing through [date removed]. I would not recommend Village Veranda for a dog let alone my loved one. It was just a bad place with horrible attitudes from the workers. They get new directors instead of new people. They are afraid to confront these workers. They have many complaints very similar to ours, but they change nothing! Please do not send anyone there. I am sorry but I needed to be honest hopefully it will help another family.

When I first brought my mom who was suffering from Alzheimer's and dementia to Creekside our biggest concern was her safety as she would escape my house while I was at work I was unable to be with her 24/7. Noted she was as fully able to care for herself when she first went to Creekside The first couple months things seemed to be going great until They failed to give her life saving medication as prescribed by her doctor. The first time she spent a week in the hospital as a result of not receiving her meds. We trusted the mistake would be fixed only for it to happen again. She spent another week in the hospital the 2nd time and needed a rehab. She never recovered and unfortunately passed away on Oct 4th from the lack of care she received at Creekside.

My loved one was previously a resident at The Colonnade at Northdale and the experience at this facility was terrible. The staff was just horrible and treated my loved one very badly. My loved one wasn't even there for a month, let alone a few weeks, before we had to move her out. They need new management and staff/people that work there as well. I would never recommend The Colonnade at Northdale to others.

Tremendous turnover, staff is a speedy revolving door. Food is pretty bad, sometimes (no kidding) a hot dog on a piece of white bread. Not the "chef inspired cuisine" in their sales pitch. No RN on staff. They don't respond to calls for help from neck worn transmitter. Executive director is as useless as a box of rocks. Housekeeping is hit or miss, went a month without any housekeeping front the canopy staff in early 2023, high turnover of all staff. They are on their fifth (useless) executive director in two years (no kidding). Two executive directors have been fired in the past year ([dates removed]) one for stealing funding from residents. Please feel free to inquire about this statement with their sales staff. Sadly, I made a mistake placing my loved one in this facility, I thought they would be better, or at least adequate. Unfortunately, new and shiny doesn't mean "good". Don't believe the sales pitch from SRI (lack of) management - the canopy at Walden Woods. STAY AWAY!

If we could give all 0s we would. This was a huge disappointment. When I was visiting, I was told so many impressive things. In reality, they didn't exist. The cleanliness of the community was adequate, although some of my mom's clothes ended up missing from the laundry and she ended up with some things that weren't hers, I visited almost every day and not one staff member introduced themselves nor went out of their way to visit with me. It seemed the staff were busy either on their phone or visiting among themselves. I was very upset to find my mom bruised and tore up from falling and not being notified, this was the case not just once but several times. I, also couldn't find anyone who could tell me specifically about what happened. Very upsetting to find out accidents aren't recorded. Apparently, nothing is. Many times when I came, she was sitting in a diaper with diarrhea. When I told they only needed to give her the Muralax once a week , they continued giving it every day. Meals/Dining were fine but staff would have everyone sitting at the tables, usually way before the meals arrived. For activities, up until the last week, there weren't any. There was a calendar that listed each days events but they didn't happen. Very misleading. The only activities that my mom got was sat in front of the television. Many times without her glasses on and in a wet or poopy diaper. I would not recommend this place at all. I was told so many things that weren't true. I was told that each person had a schedule that included bathroom breaks and walking, never happened. I was told she would go on field trips, never happened. I was told she would be involved in activities, never happened. No individual learning plan/activities, no data collected on accidents or medical given or overall daily information, no communication for anything. Very bad experience.

My mother is 89 years old. She rented an apartment in the independent living section and many promises were made. There was constant problems with the food services that were promised, she spent three days in her apartment with NO heat in December and no one in the building could fix it or get anyone to come fix, and many other issues with the construction of the vents, doors, and windows. In addition, her dishwasher did not work, they could not figure out why. She asked them to pull it out and they discovered it NEVER HAD AN ELECTRIC CORD to it. She was promised a face to face meeting with someone from corporate on a Tuesday. Never happened. My mother moved, I called and told them the issues and why she was moving. In addition, I requested they not charge her the two months notice, due to all the issues and was promised a call back after they spoke with management that DAY. Never happened. Now, they are trying to collect over $[removed] from my mom. Do not rent from these folks.

Harborchase is the worst place ever, bar none. Non existent on-site management. Patently mean staff and CNAs. When I say "mean," they were profoundly mean. Using loud abusive language, withholding their name tags from me, sometimes purposely giving me bogus names. Nonchalantly abusing my affected limbs out of exasperation, chastising thinking I should be able to move them. Sometimes I actually thought I was in hell. Now I'm in a different place that is 100% better than Harborchase!!! Harborchase has good dining, good food. But the staff is beyond horrible. I encountered 2-3 kind aides. The rest were not.

The Canopy at Duval Station may appear real shiny at first glance but as we all know looks can be deceiving. The very same “memory care” facility that LOST my grandfather months ago recently kicked him out in the middle of the night (without any warning) for exhibiting the characteristics of a typical dementia patient. Their staff member lied about the incident and it wasn’t until the family insisted on seeing video proof that the truth was brought to light. Please, if you are considering trusting The Canopy with the care of your loved one, be very leery and watch for red flags so you don’t have to endure the same heartache my family has.

The aids were not courteous and when my parents pushed the button for assistance they had to wait sometimes 2 hours for help. We would not recommend this facility to anybody. Their billing department was inadequate and continued to charge us months after we moved out. Very poor management

I would not send a dog there!! The nursing staff for one has to care about the residents and act like they care. To leave a resident on the floor without going to them and to check if they were injured is criminal! Or to make sure residents are hydrated would be nice for the poor souls who were asking for water!! Staff was truly evil!! Others have told me the same evil practices were done when they were there!! The other day we were at the Veranda no one came into my loved ones room until 9:00 when I called! They are diabetic and needs insulin and food first thing. Girl said our loved one was just going to have to wait until seven other people were cared for because they were short staffed! The Village Veranda is a lovely place it will be stellar with a medical staff who truly cares for the residents! They need more staff and professionals who want to be there and are kind to the residents!

Facility smells like pee every where because they don’t care about their patients for example [name removed] who was laying down on his own pee for god knows how long poop stains on the ground. I would tell others looking into Heron House to be aware of the level of care that is necessary for your loved one. Also - make more than one evaluation. Based off of our experience with this facility, I would not recommend them to others.

No one ever answered the phone not even hospitals could get thru. The caregivers are not trained. They don't know how to assist my loved one in showering. The private Occupational Therapists we've brought in are having to train Indigo's care staff. Instead of showering my loved one twice a week like they're supposed to, they're only doing so once every two weeks, yet they're charging us for extra services. He needs assistance getting dressed, and sometimes they wait so late to help him that he misses breakfast. He's a diabetic, so that is not okay and shows a lack of organization and planning. They said they hired a new activities director, but their idea of an activity is getting everybody together to watch Jeopardy. Initially they promised off-site activities, and there have only been two since the beginning of October. There are minimal activities during the week, and no activities and low staff coverage on the weekends. I wouldn't recommend anyone go to Indigo. We are just not seeing results after bringing our concerns to them. The only good thing I can say about the place is the food service is good, and the rooms are nice.
