The staff has a lot of turnover and a lot of incompetence. The administration is noncommunicative and secretive. We've caught them in lies. The day before I went to Florida to bring my mother up to assisted living in NY my husband and I signed the paperwork. At that time (and not ever before) they informed us of resident deaths from legionaires disease a few years prior. I discovered they were overcharging my mother for months last fall. The health office is VERY unorganized. The med techs made several mistakes. The in house PCP phones it in, so we're moving my mother to an outside provider. My mother had cash stolen from her room by staff last fall. They had bed bugs in the facility a couple of months ago, but lied to residents and families about it.

Consider it for Medicaid funded, but if someone has private funds look elsewhere. Food - Just not very good. For example at dinner (evening meal) they serve soup and sandwiches. Activities - No outings with transportation despite saying that on tour. Also their communication, med management and health management is a chaotic mess. Impact examples are: • Not sending records to specialist for appointment resulting in waste of time and money for that visit. • Giving my mother someone else's meds (at least twice) which my mother caught.

Yes they need to have a dietitian. The food is fried and canned. They could do a lot better. Not to mention, that there are no guests allowed at dinner time or any other time in the cafeteria. Does not make it nice for visiting family that they can't dine with their relatives

My loved one was previously a resident at The Promenade at Middletown and her experience at this facility was overall just okay or less. Unfortunately, she was not happy at this facility and we moved her to a different place.

The facility itself appeared to be above average compared to some others I saw. They had a beautiful outdoor seating area that I liked. We are country people and we thought the location was not very safe, but for someone who lives in the city they might not even blink an eye. I did not like the shared room set up. It was more like a hospital and the residents have to share the same bedroom rather than having separate bedrooms and a shared living space. It was just not the right fit for my dad.

we toured the Promenade at Middletown and upon first impression, I thought that it needed a renovation. although the rooms looked nice, it just seemed very dark. we chose another community because the Promenade did not have the quality of living that we were looking for.

This community seemed dark during our tour. There were not a lot of windows to let light in. It is an older building. The community was clean and the people we meet were nice.

The food is awful. She didn't like any of the activities but they did have a lot to do. There were a lot of people who stayed in their rooms. It was very well maintained, and it was very nice. The director seemed to have an attitude. They wouldn't let them have a microwave in their room and said it was a law which it wasn't because I looked into it.

My tour of this community was decent. The community could use some updating. From what I saw the community is clean, but it could look better. The staff I met was kind and friendly. They didn't offer many activities, which was disappointing. Overall this is a decent community, but could use some improvements.

Where do I start? Between a lack of overall cleanliness, loss of medical records, failure to provide timely physical therapy, etc. Unfortunately, we are stuck here for now. Luckily, my Mom has the nicest room in the place. It better be for $[removed] a month. Oh and the food is pretty bad, too.

They had 2 bouts of bed bugs during our stay here. They had mice after a while as well. They were several residents I saw who who were under - nourished. There was one person who was terrific with activities, but he was only there 2 times a week. They had people working in the kitchen who were stealing food and taking it home. Some of the aids were turning off the intercom so they would not get beeps during the night from the residents who needed help. They only had one aid during the night for 50 residents. I would absolutely not recommend this facility to other families.

Run the other way! The place smells and it's really old and outdated. They have no security set up after 6pm - anyone can wander in or out! There are no security cameras, and residents can just wander the hall. What a bunch of [language removed] artists who blow smoke up your bum and let you think everyone there is so caring, when they are NOT. It's a business and a place that just wants to make money. You have to chase after them to make sure they take care of your relative as they will do the least amount of work they can get away with! The hallways wreak of urine. The food is [language removed] (they claim it's "gourmet"). The staff spends a lot of time sitting around and talking and not a lot of doing. My relative is a bit hard of hearing and couldn't understand what they were telling her to do, and when she asked them to repeat themselves, they were already down the hall onto the next one. [Language removed]? You get to spend about 2 minutes with the in-house doctors. Find another place.

A friend recommended this place to me for my dad. As I was told, this was once owned by a different party, and just recently was bought out by a new company. What started out with a $1000 "non-refundable community fee", now totals $173 per day for a regular private room on the second floor. The problem I am currently having after just 5 days of my dad living there, he still was not provided a simple shower curtain in his bathroom. I was quoted the "published pruce" of $105 per day for this room. They than tack on $20 per day for medicicine management, and recently added $40 per day for "level 2 care". Now the price is $113 per day (I have still yet not figured out why I was quoted on paper $105, that turned into $113. Now the problem I have with the "level 2 care " for the extra $40 per day seems to be where they are charging extra for something they claim he needs, but I am puzzled as to what they are providing for thr extra $40 per day. When I visited him today (5th day being there) I decided to use the bathroom in his room and noticed STILL NO SHOWER CURTAIN. I asked him how he took a shower with no shower curtain, and much to my disappointment his reply was that he has not had a shower yet. There is a trust level that has now forced me to explore other places. Also, the residents were served a HAM AND CHEESE SANDWICH for dinner today. Do some reseaech first. Be sure to ask "what are the extra charges?".

My mother has dementia I was told by [name removed] (Director of Community Relations) 1-They would make sure my mother was at every meal (they didn't) 2-She would be brought to activities (didn't happen) 3-She would be showered on a schedule (not true) 4-Due to her dementia condiments were not left on her table (not true) 5-NOONE takes responsibility & blames everything on someone else 6-After 2 months I took her out The day she was leaving a resident tried to stab another resident Awful place!!!!

When you go for a tour with [name removed] you are told things that are not true. I told her my concerns with mom being there & she said they would take care of it Not true! Most of the staff are nice but every time there was a issue NO ONE took responsibility The place itself is ok clean wise but the rugs are disgusting & the ventilation isn't good I had to put a air purifier in due to dust The meals are basic not gourmet as advertised, they are basic & the residents complain it's not enough food for them My mom wasn't taken to dinner twice (that is what I observed) so I had to go there everyday to make sure she was given dinner I was told they would encourage her to go to activities but that's not true so she laid in bed all day which caused her to lose circulation which caused her to fall 4 times in 3 weeks The day I moved her out I was told a resident tried to stab another resident and he was left at the facility instead of taken out. I would NEVER recommend the Promenade of Middletown to anyone!
