I tour the facility

I did an unnoticed visit to tour the facility I was very pleased at what I saw I was greeted at the door by staff. I told her why I was there and she bothered at all. She gave me a tour of the facility and again I was pleased. The dinning was fillled with residents eating. I was offered a plate my self.i look to discuss my impromptu tour have eaten there. Needless to say my loved has a new home at transitions Oakland manor

Feels Like Family

My mom, [name removed] , has been at Transitions Healthcare Oakland Manor for several months, and I can honestly say it’s been a blessing. Staff are kind, attentive, and treat her like family. They go above and beyond to make sure residents are comfortable and engaged, and there’s always something going on, like crafts, music, or bingo. The building is clean, welcoming, and really well-kept. Communication with families is great too thanks to [name removed] follow-ups. If you’re looking for a place where your loved one will be truly cared for by people who love what they do, Oakland Manor is it.

Visiting a friend

On a recent visit to the facility i was struck by the friendly environment and cost atmosphere. The facility was clean and the residents were well cared for. Several residents made a point to welcome me and my friend introduced me.to many new people that are now her neighbors. The Activity person, who I understand was new, was keeping everyone engaged and entertained. The Director was.active and engaged. I was very pleasantly surprised.

Grateful to have found a place for our Mom

After careful consideration and much prayer, we felt like God lead us to Transitions, and we have been very satisfied with our Mom’s care. She enjoys the activities and raves about [name removed] who coordinates the schedule. It is obvious that the nurses genuinely care for the residents and that the residents care for each other. [name removed] has been quick to answer all of my questions and is very approachable. If there is any room for improvement, it would be the food.

Good experience with Transitions

At Transitions they have taken good care of my mom. They are very attentive to my mom's health and they follow up to make sure things are taken care of .It is a nice place for the money. I recommend it to anyone looking for reasonable care options. The room layout is like a hotel. They have done a good job during the Pandemic managing all the residents and keeping them safe.

Excellent care!

Our experience with Humbert Lane Pesonal Care home was a very positive one, overall! We initially used them for skilled nursing care after a fall, and I thought they did a great job. We ended up sticking with them and using their personal care service, which is also very nice! The staff is friendly and caring, and the facility is clean and well kept. I would highly recommend them!

LOVE IT!

My father has been there for 4 years. I love the new ownership and the director really cares about everyone! The new ownership has done huge improvements to the building and the care is good. Even my sisters and I were unable to to provide perfect care, if I have issues they do their best to resolve it quickly and even thank me for bringing it to their attention. My dad has been in a nursing home twice in the past and the care he gets here far surpeeds the care he ever got there and they had RNs around the clock. Not sure when the terrible review occurred but I totally disagree. I visit several times a week and am always greeted with a warm smile and I have no problem leaving him in their care. The staff works hard and always goes the extra mile. I see very few visitors when I visit and see that the staff is just like family to all the residents. The prices are fair, especially when I consider the amount of care my dad is given. He is always clean and although he cannot verbally communicate, he seems content and smiles a lot. As my Dad's condition worsens and he ages, they always try to put things in place to be able to provide care. I can't say enough good things but could probably write a book!

Happy!

Found this place after much research. Glad to know that they will not just kick my mom out when she runs out of money. A friend found this place after she spent all her moms money at a pricey place. I visited her and have seen the new owners continue to make things better.it was a no brained when I needed help with my Mom.They have explained what happens and even given me Medicaid information so I can start keeping track now. It's no castle but it has all the things my mom needs, good care from people that really care, good meals and activities. She is always clean and seems happy.

GREAT!!

The care my Mom recieves is by far better than I ever expected. The staff is warm and friendly and always seem to go the extra mile. My Mom was in an assisted living that almost took her for broke. the care she gets here is affordable, without all the extra bling that she would never use. If I have issues it is always handled immediately and the director always thanks me for bringing it to her attention.

Had to visit a resident for a friend. Very hospitable environment. Staff very friendly.

Great staff. My friend is thriving there as opposed to the nursing home. It is a clean and comfortable facility with great activities. It is also a very diverse community and the residents seem to care for one another like friends.

Employees are loving and kind

I saw first hand how very hard it is to be a professional caregiver. Especially with my Moms version of Altzheimers. They are respectful of her even when she's difficult. She hasn't been there very long, but I see how the employees are when they don't know you are there. Loving, caring and kind.

I Feel Better at Ease in this Community for my Loved One

My loved one is in a private room. There a lot of nurses, physical therapy, skilled nursing on the second. I would recommend this community. My loved one loves the food. You can schedule a visit with your loved one to see them in person in the dining room. You do have to schedule this visit 48 hours in advance. You do have to fill out a piece of paper, have to have your temperature taken, stay approximately six feet away when visiting.

Comfortable with the care our loved one has received.

Our loved one has been under hospice care for awhile now, and we feel that they is getting much better care being under hospice then he was at Transitions. Our loved one is in a private room which we are very pleased with, they also have their own bathroom which we are very happy about that. Their room is very well suited for their needs. When we were first looking at this community the staff was very accommodating and kind. It seemed to be a good fit for our loved one, and that they would be very comfortable. It is a smaller facility which we thought they would enjoy that
