Caregivers kind and caring. As with all such facilities they have a difficult time maintaining sufficient number if cnas. Facility is beautiful with ample room.for meeting with guests

Would not recommend.

I would have given this a 1 except there was a short amount of time/people mixed in that were good. But overall I felt like I failed my mom by picking this place. Everything started off great but once I wasn't there all day it got worse. There was always soiled stuff in open trash cans no matter time of day. My mom, her clothes and bedding were almost always dirty unless I was there on a party day or the director was taking a group through. Again would a 1 but there were a couple part time agency people that were awesome, friendly and professional. The people at the front desk were the friendliest and most helpful. I could walk through the care unit and no one would speak. On more than one occasion I overheard people saying I am not going to do something that was part of their job. There were some agency people who would sit or wander and not do anything, but the friendliest people there were a couple of the agency people and the all hospice workers. Moms bed was often dirty, she was often dirty and I always had to request she be bathed and her bed changed. workers would refuse to help her eat; there were times when workers would disappear and no one knew who was supposed to be watching the group; workers never disciplined or removed after many infractions; more than once saw a worker asleep and she was only one on duty. I finally put a camera in mom's room and that was only way I could get her changed on a regular basis. Something was going on with the nurse : she had a family issue, she had Covid but then she didn't, some other issue- rumor was she was looking for a job. Didn't have a nurse onsite for weeks at a time. My mom had the best care by the hospice workers. the best food was served at the family parties. For a few weeks they had a wonderful activities person who worked with everyone at every level of the disease and she was always there doing stuff. She was friendly, greeted all the visitors and engaged with everyone. She was promoted and they moved her to assisted living and then the memory care people spent months parked in front of the tv for a lot of hours. Much too high for the poor care.

APFM needs to stop recommending them, they do not use your company anymore for referrals, but they take your people coming from your website, please take them off your website. Since ALG has taken over and they don't have enough help, they are using cooks and activities personel that have no experience, and it makes APFM look bad. Remove the. Now!!

Tour of Village on the Park

We did not have a good tour experience with Village on the Park. We literally walked in and left within 10 minutes. It is very dark and small, the vibes were not good. The receptionist was on her cell phone and did not acknowledge us for several minutes. Our loved one had an emotional support dog and they told us they would not allow the dog. I would not recommend based on our experience.

My mom has been here in Independent Living for just over two years. Initially pretty good but has gone steadily downhill. Complete change of staff several times over including at least 3 managers. The latest is another “interim”. In the dining room there are NO provisions or choices for heart-healthy, low fat or low salt diets. There have been 5-6 chef changes in the time she’s been here. The one good one did not stay long. Despite what we were told initially and is still listed on their website, there is no “12 hour continuous dining”. Breakfast is 7:30-9. Lunch is 11-1:15. Dinner 4-6:15. Other than those hours the dining room is closed and residents line up in the hallway waiting for it to open. Come later and items may not be available. And the rules change with no notice. Apparently as of today, a meal for a visiting guest or family member requires an $8 cash payment at the table instead of being charged to the resident’s account by voucher. My 95 year old mother was very upset and left without eating. Poor response for maintenance despite multiple calls and visits to the front desk. Yes, the apartments are larger than at other facilities but had we known then what we know now, we’d have made a different choice. Do yourself a favor and look elsewhere.

village on the Park not suitable.

Village on the Park was not acceptable to us as they wanted us to bend to their schedule and we preceded to have them bend to ours. Their food was baseless and uninspired so we are out every night. Their help were pleasant and mind but we're hampered by restritions. We spent two weeks there and that was 10 days to long.

Alarm button lousy

My mother was left on the floor of the bathroom after a fall in the night. Their call system must have been broken. She even broke a finger nail from pressing the call button so many times. She was not found until her home health assistant came to help her dress at 7:00 am the next morning. They could never produce a log for us to see when calls had come in during that night. I wonder if they really have one. Then we had to wait the standard 60 days to move out. The food was pretty good most of the time.

Not worth the money

My mother has been in the Memory Care section for about a year. Most of the staff are friendly and helpful (I certainly couldn't do that job), but that’s about the best thing I can say. The activities calendar is a joke. The food is dismal, unappetizing and not nutritious. Supplies are consumed at an unbelievable rate (1 gallon of laundry detergent and 75 bed pads in one week). Clothes/sheets/towels are frequently lost. “I don’t know” is generally the answer to most of my questions. Staff turnover is high; every time I visit there is a new person and someone else is gone. I expected a higher level of professionalism and service for $4,800/month. I'm sure there are worst places than this, but there are probably better places for the money.

Keep looking if you require extra care

My grandfather is at Provident Village currently, and we are currently looking for a new place for him. He has been there for a few years now, its only until recently, under new management that things have become so bad. He is on hospice and requires a great deal of care, which meant we were allowed to see him regardless of covid-19 with restrictions which I can acknowledge are necessary. Since March he has lost 30lbs because we cant go to the cafeteria with him to feed him his meals. We were bringing food for him and now they are no longer letting us do that which is outrageous considering they are not making sure he eats. when we would visit he would not always have his oxygen on and now I'm afraid he isn't getting it as prescribed. These and other issues is why I am giving Provident Village such a low score. Cost is $5400

Do not recommend this Facility.

Unprofessional and dirty Memory Care floor. Did not receive the care promised nor attemp to make anything right. Moved our mother. Gave 30 day notice after only 15 days and lost the $2000 facility fee you are charged when you move in. That says a lot about the conditions of this facility and staff.

Not so happy with this place

The facility is new, but there is already evidence of structural problems with the building, however, it is very clean and neat. This facility is not well staffed and you will hardly ever see any staff around. I worry about the safety of the residents. I have witnessed the director of the facility be rude and mean to the residents. I had to talk to the people who work there about doing more for the residents because the activities and outings are almost non-existent . It has gotten a little better since.
