Care Partners Assisted Living

I am happy with the needs for my father. The community is nice with good staff members who are caring and friendly. They do offer activities, but he is not participating at this time. The community is clean and well maintained. The menu is appealing. I would recommend the care and value to other families for a loved one.

One issue, I have encountered is reaching someone after hours, which can be difficult. They should offer services without being asked. one time, my loved ones laundry was not put away for a week or so, and was just sitting in the basket. There needs to be more communication between the different shifts, which would be nice. Overall, not being able to reach anyone in the evening, is my only concern. The phone just rings and rings, and there is no answering machine.

A few people on the day shift are amazing, one on the night shift is good. Meals are often late and not appealing. I can attest to the quality of the meals, as I've eaten lunch and dinner a few times.

Before I brought my mother up here from Texas, I looked at a lot of places. I had only a few days to find options for my mom. Country Terrace was able to work on our timeline. They have done a good job with Covid-19. They have a high rate of staff turn over. The activities are limited. I don't think socialization and activities are high on the priority list. The meals are not up to standard it is more like sandwiches and hot dogs not healthy meals for seniors. It is hard to really tell with the lockdown due to Covid-19.

There was very little care help on duty. We even had to have a care helper give us a tour. We decided this was not a place for us.

Unfortunately, I had to move my Father out of this facility. I felt like this community didn’t fit him very well. There was something always lacking and it was never personal at all. It seemed like if you didn’t fit into the group, you were on your own. I also didn’t like that the individual attention was lacking. My dad is still very active and they would never take him for a walk. In October there were a few days that got into 80-degree weather and I went to the gate to take my Father for a walk, and it was locked. The facility seemed like they wanted the residents who were slower paced. Don’t get me wrong, the facility was really nice and the food was great but, they had no regards for things like laundry. My dad had bedding and clothing that didn’t even belong to him. I would recommend this facility to families who want a slower paced facility.

We had moved my father into this community, but he was not there long. The staff was nice and we were happy with the cleanliness. Activities are offered here. Families are recommended to see this community to make sure it is what they are needed for the care of a loved one.

I would not recommend the community to others. The food is of poor quality and options are limited. There is a lot of canned vegetables, and not enough healthy options. The manager isn't around to speak with if you have questions or concerns about your loved one.

the activies were great when they have them. cant always find a person to come in,,the bingo games are fun with nice prizes of good quality..the painting projects were enjoyablethe .funiture is dirty, at least the arm chairs look very dirty..go sit on one. the food is ok one day and then the next it can be total junk. staff on phone while doing cares. i saw one staff member vaping in the building..its the same as smoking if you ask me. stopped in after dinner one time and floor was not sweep and tables were smeared with food while residents sat at them visiting with each other. staff didnt look busy, so why didnt they clean off the tables?the staff try very hard to please. managment changes like the weather. but my family member was happy.

We toured Care Partners while looking at options for my husband. The place was nice; it was a smaller setting with fewer people than some of the other communities we were looking at. We would have been interested in moving in, but the administrator who helped us was very nice, but was disorganized, ill prepared, and did not follow up with us. We decided to look elsewhere.

I was not impressed with my tour of this community. I walked through the community by myself, and nobody stopped me. Nobody approached me till I was leaving the community, and I didn't feel that was right. If I could just walk into the community, that means anybody can. It wasn't the facility for us.

I am looking to move my 90 year old mother from assisted living into a memory care facility due to her paranoid/dementia getting worse. Friday, 3/30/18 at 10:31 I placed a call to Care Partners and set up an appointment for Monday, 4/2 at 10:00 to visit the Hortonville, Wi facility. I gave them my name and my telephone number and was told at that time that they would see us on Monday. We drove 1-1/2 hrs from the south side of Milwaukee to keep this appt. Upon arrival (approx. 10:05) we were greeted at the front door by the receptionist who stated that she would be the one to show us around since the coordinator did not have time to talk to us. ?? I thought we had an appt. I wanted prices, etc. How am I to decide what to do with my mother if they can't take a few mins. to talk to us. I was told in no uncertain words that I would have to make another appt. ?? What don't I get?? Evidently this facility even though its new needs a new leader, this one is not working well. If they didn't have time to see us why didn't they call me and let us know so we could change the day instead they would let us drive all that way. I'm sure by now you've figured out that I would never send my mom here and I will never recommend this facility to any others.

The facility is very old and lacking in maintenance. Requested grab bar in bathroom has not been done in 7 months, resulting in a fall. Management and supervision of staff is lacking. I have observed Assisted Living standards being ignored by CNA staff who regularly bring their children to work. The staff is unhelpful and at times disrespectful to residents. Avoid the facility if you can do so.

Wouldn't recommend......... Dishes aren't properly washed just sanitized. I wouldn't recommend to anyone staff are lazy!!!!!!!!!!!!! !!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

My husband had been in assisted living for 3 months when they watched him walk out the door and not come back. He had dementia as well as other medical issues. They did not put out a silver alert for him or even look for him till after 10 pm, 5 hours after he was seen leaving. They called me at 10 pm and asked when I was bringing him back. I was horrifief! I never took him. out that day at all. they found him next morning dead across the street. The police never came looking till morning. When my hmo person was there trying find out what was going on. I would not trust another living being there. Lack of staff response and lies told by administrator.

Very poorly managed. Care Staff is very rude to residents and families. While the building is very nice and spacious, it does not make up for the poor quality of care.

My mother was a resident of Care Partners on French road. During the 8 months that she resided there, there were 3 managerial changes, staff came and went on a weekly basis. Simple, basic every day needs were not met, depends were not even changed every day! This facility is beautiful, but it is severely understaffed, and the staff is not trained to handle dementia patients.

My father was here. He wasn't showered for eight days. Medication errors. He vomited in bed and bedding wasn't changed even four days later. Staff [Removed] our family. I have read the guidelines.

The Country Terrace in Stanley has less than optimal care for the residents. Director is the most uncaring person along with most of the rest of the staff. No activities, no resident interaction, no snacks offered even though this us what is told when you move your loved one in there. Workers spend more time on checking cell phone and on smoke breaks than taking care of residents. I would not recommend this place to any family considering putting there loved one at this assisted living.

Please do not leave your loved one here. Do not let the new building fool you...keep looking. Staff not properly trained, staff over worked and complaining. Of course there were a few staff members that cared and were good employees, but not many. Many times there were 1-2 staff on shift that could not dispense medication! Our loved one had to wait until next shift to receive pain mediation! Actually found staff sleeping during work hours at different times! Families were not told of negative events of loved ones that took place while they were gone, that were witnessed by other family members of other residents. Staff had left the building unattended at times, and I could go on and on!! Please take my word for it and keep touring other facilities. You do not want our loved one here. After removing our family member we found out other terrible things from previous staff and other families that were familiar with owners reputation of sweeping things under the rug. Wherever you leave your loved one, you have to visit often and not take anything for granted. It is hard enough moving them into such a facility, but you want to know they are safe and well cared for. Talk to other family members before making your decisions. You can look at this facilities public records to see their violations of state codes.
